目的:探讨老年人群基线维生素D3与心血管疾病发病风险的关联及其强度。方法:本研究利用中国老年人健康长寿影响因素调查(CLHLS)数据库,入选2011—2014年调查的≥60岁老年人1 383例,年龄(84.9±12.1)岁,男性662例,女性721例。队列人群共随访4 149人/年,每例随访者平均随访3年。随访结局为维生素D3与心血管疾病发生的关联及其强度。结果:1 383例非心血管疾病人群经3年随访发生心血管疾病196例(14.2%)。调整年龄、性别、体质指数、吸烟、饮酒、总胆固醇、高密度脂蛋白胆固醇、收缩压、空腹血糖、尿酸等因素后,Cox比例风险模型结果显示,维生素D3水平每升高1 mmol/L,心血管疾病发病风险下降2.0%(n HR=0.98,95%n CI:0.97~0.99);60~79岁、80~99岁和100岁及以上老年人群的心血管疾病分别下降1.9%(n HR=0.981,95%n CI:0.968~0.994)、2.8%(n HR=0.972,95%n CI:0.956~0.989)和10.5%(n HR=0.895,95%n CI:0.830~0.965)。n 结论:维生素D3与心血管疾病发生相关,老年人群维持较高水平的维生素D3可能是预防和控制心血管疾病的有效措施之一。“,”Objective:To investigate the association of baseline vitamin D3 with the risk of cardiovascular disease and its strength in elderly populations.Methods:This study used data from the database of the Chinese Longitudinal Healthy Longevity Survey(CLHLS). A follow-up survey was conducted on an elderly population aged≥60 years from 2011 to 2014.A total of 1 383 subjects were entered into the final analysis, aged(84.9±12.1)years old, including 662 males and 721 females.The cohort was followed up including 4 149 visits/year, with an average follow-up of 3 years for each participant.The end-points for followed-up were the association of vitamin D3 with cardiovascular disease and its strength.Results:Of 1383 individuals without cardiovascular disease at baseline, 196 cases had cardiovascular disease after 3 years of follow-up, with a cumulative incidence of 14.2%.After adjusting for age, gender, BMI, smoking, drinking, total cholesterol, high-density lipoprotein cholesterol, systolic pressure, fasting blood glucose and uric acid, Cox proportional hazard model results showed that for every 1 mmol/L increase in the vitamin D3 level, the risk of cardiovascular disease decreased by 2.0%(n HR=0.98, 95%n CI: 0.97-0.99)in the whole population, 1.9%(n HR=0.981, 95%n CI: 0.968-0.994), 2.8%(n HR=0.972, 95%n CI: 0.956-0.989), and 10.5%(n HR=0.895, 95%n CI: 0.830-0.965), in the 60-79, 80-99, and 100-plus age groups, respectively.n Conclusions:The vitamin D3 level is associated with cardiovascular disease.Maintaining a high level of vitamin D3 in the elderly may be one of effective measures to prevent and control cardiovascular disease.
