目的检测膜联蛋白Ⅰ(Annexin Ⅰ)在正常鼻咽黏膜上皮组织(NNET)、原发鼻咽癌(NPC)组织及颈淋巴结转移鼻咽癌(LMNPC)组织中的表达,探讨其临床病理学意义。方法采用免疫组织化学S-P法检测30例NNET、98例原发NPC组织(高分化鳞癌5例,中度分化鳞癌13例,低分化和未分化癌80例)及20例LMNPC组织中Annexin Ⅰ蛋白的表达情况;采用SPSS13.0软件包对数据进行非参数检验。结果与NNET比较,Annexin Ⅰ在原发NPC组织中的表达明显下调(P<0.05);与原发NPC比较,Annexin Ⅰ在LMNPC中的表达水平明显下调(P<0.01);98例原发NPC组织标本中,Annexin Ⅰ的表达水平与NPC的病理分化程度、临床分期、复发及局部淋巴结和远处转移有关,Annexin Ⅰ低表达的肿瘤较高表达的肿瘤分化差,有更晚的临床分期、更容易复发、更容易发生局部淋巴结和远处转移(P<0.05 or 0.01)。结论Annexin Ⅰ与NPC的分化程度、转移和预后相关,有望成为预测鼻咽癌转移、预后和区别NPC分化程度的分子标志物。提示Annexin Ⅰ在鉴别NPC组织学分化程度、预测鼻咽癌病人预后及预警鼻咽癌转移和复发方面有一定的临床价值。
Objective To detect the expression of annexin Ⅰ in normal nasopharyngeal mucosa epithelial tissue (NNET), primary nasopharyngeal carcinoma (NPC) and cervical lymph node metastasis of nasopharyngeal carcinoma (LMNPC), and to investigate its clinical pathology significance. Methods Immunohistochemical SP method was used to detect the expression of Annexin in 30 cases of NNET, 98 cases of primary NPC (5 cases of well-differentiated squamous cell carcinoma, 13 cases of moderately differentiated squamous cell carcinoma, 80 cases of poorly differentiated and undifferentiated carcinoma) and 20 cases of LMNPC Ⅰ protein expression; SPSS13.0 software package for non-parametric test data. Results Compared with NNET, the expression of Annexin Ⅰ was significantly down-regulated in primary NPC (P <0.05). Compared with primary NPC, Annexin Ⅰ expression was significantly down-regulated in LMNPC (P <0.01) The expression of Annexin Ⅰ was correlated with the degree of pathological differentiation, clinical stage, recurrence, and local lymph node and distant metastasis in NPC. The higher expression of Annexin Ⅰ was associated with poorly differentiated tumors and a later clinical stage, Easier to relapse, more prone to local lymph nodes and distant metastasis (P <0.05 or 0.01). Conclusions Annexin Ⅰ is correlated with the differentiation degree, metastasis and prognosis of NPC, which is expected to be a molecular marker for predicting NPC metastasis, prognosis and differentiation of NPC. These results suggest that Annexin Ⅰ has certain clinical value in distinguishing the degree of histological differentiation of NPC, predicting the prognosis of patients with NPC, and predicting the metastasis and recurrence of NPC.
