目的为了解伊犁州经性感染艾滋病病毒(HIV)者的性伴类型及其分布特征,为制定符合当地特点的HIV经性传播预防措施提供依据。方法对伊犁卅六县市自2011年以来报告的所有HIV感染者/艾滋病(AIDS)病人(3527例)开展传播途径调查,对其中经性传播者详细询问性对象及性行为细节,并判断其获得感染的性伴类型。结果 1661例异性性传播中阳性配偶占51.2%,固定性伴占14.0%,临时性伴占17.6%,商业性伴占17.2%;46例同性性传播中临时性伴占76.1%,固定性伴占13.0%,商业性伴占10.9%。异性性传播中不同性别、不同年龄段、不同报告年份性伴类型构成有所不同:女性以阳性配偶和固定性伴为主,分别占69.8%和14.2%;男性以商业性伴和临时性伴为主,分别占40.4%和26.8%。高年龄者经商业性伴感染占42.7%,远高于低年龄者的15.8%。阳性配偶比例呈逐年下降趋势,由2011年的56.5%下降至2014年的35.5%。结论伊犁州HIV异性性传播的性伴类型以阳性配偶和固定性伴为主,同性性传播以非商业临时性伴为主。预防异性性传播,应重点关注女性经配偶或固定性伴感染,同时不应忽略老年男性群体。
Objectives To understand the sexual characteristics and distribution of sexual partners of sexually transmitted infections (HIV) in Yili Prefecture, and to provide a basis for the development of HIV transmission prevention in line with local characteristics. Methods A total of 3527 AIDS patients (3527 cases) reported from six counties and cities of Yili County since 2011 were investigated for their transmission routes. Sexual intercourse patients were asked about the sexual particulars and sexual behaviors in details, Obtain infected partner types. Results Among the 1661 cases of heterosexual transmission, 51.2% were male, 14.0% were persistent, 17.6% were temporary and 17.2% were commercial. Of 46 cases, 76.1% had temporary sexual partners, Accounting for 13.0% and commercial partner accounting for 10.9%. Heterosexual transmission of different genders, different age groups, different reporting years, the types of companion types are different: women with positive spouses and fixed partners, accounting for 69.8% and 14.2% respectively; men with commercial partners and temporary partners Mainly 40.4% and 26.8% respectively. 42.7% of older persons suffered from commercial partner infection, much higher than the 15.8% of those with lower age. The proportion of positive spouses showed a declining trend year by year, from 56.5% in 2011 to 35.5% in 2014. Conclusions Sexual partners of heterosexual transmission of HIV in Yili prefecture are mainly positive mate and fixed partner, and homosexual transmission is mainly non-commercial temporary partner. Prevention of heterosexual transmission should focus on female spouses or nosocomial infections, and should not neglect the elderly male population.
