目的:探讨复方丹参注射液联合血液透析对终末期肾病合并心力衰竭患者临床疗效和氧化应激指标的影响。方法:选取终末期肾病合并心衰患者72例,随机均分为对照组和治疗组,对照组进行常规治疗,治疗组在对照组基础上使用复方丹参注射液进行治疗,观察两组患者的总体疗效、心功能、左心室形态、活动能力、氧化指标及药物安全性。结果:治疗后治疗组显效率(38.9%)和总有效率(83.3%)均明显高于对照组,差异有统计学意义(P<0.05);与对照组比较,治疗组左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、室间隔厚度(IVST)、左室后壁厚度(LVPWT)均显著减小,左室射血分数(LVEF)及6 min步行距离均显著增加,差异均有统计学意义(P<0.05);与对照组比较,治疗组血浆中丙二醛(MDA)含量明显下降,超氧化物歧化酶(SOD)、过氧化氢酶(CAT)、谷胱甘肽过氧化物酶(GSH-Px)活性明显上调,差异均有统计学意义(P<0.05);两组药物治疗安全性比较,差异无统计学意义(P>0.05)。结论:复方丹参注射液联合血液透析治疗终末期肾病合并心力衰竭效果显著,可改善患者心功能,减轻氧化应激损伤。
Objective: To investigate the effect of compound Danshen injection combined with hemodialysis on clinical efficacy and oxidative stress in patients with end-stage renal disease complicated with heart failure. Methods: Seventy-two patients with end-stage renal disease complicated with heart failure were randomly divided into control group and treatment group. The control group was treated routinely. The treatment group was treated with compound Danshen injection on the basis of the control group. Efficacy, cardiac function, left ventricular morphology, activity, oxidation and drug safety. Results: After treatment, the effective rate (38.9%) and total effective rate (83.3%) of the treatment group were significantly higher than that of the control group, the difference was statistically significant (P <0.05); compared with the control group, (LVESD), LVEDD, IVST and LVPWT were significantly decreased, left ventricular ejection fraction (LVEF) and walking distance at 6 min were significantly increased, (P <0.05). Compared with the control group, the content of malondialdehyde (MDA) in the plasma of the treatment group decreased significantly. The contents of superoxide dismutase (SOD), catalase (CAT) The activity of GSH-Px was significantly up-regulated with statistical significance (P <0.05). There was no significant difference in the safety of drug treatment between the two groups (P> 0.05). Conclusion: The compound Danshen injection combined with hemodialysis treatment of end-stage renal disease with heart failure effect is significant, can improve cardiac function and reduce oxidative stress injury.
