目的 探讨达纳康对糖尿病视网膜病变 (diabetic retinopathy,DR)视网膜光凝术后眼前段改变的干预效果。 方法 采用前瞻性随机对照方法将 72例 DR患者的 72只视网膜光凝治疗眼 (DR ~ 期 )分为达纳康干预组和对照组。应用超声生物显微镜 (ultrasound biomicroscopy,UBM)观察和定量分析两组患者视网膜光凝术前、术后第 3、7d前房深度、前房角、睫状体和前段脉络膜的改变。 结果 术后第 3d,对照组视网膜光凝术眼眼压升高 ,与术前相比差异有显著性意义 (t=- 2 .82 1,P<0 .0 1) ;除鼻侧前房角外其余象限前房角较术前变窄 (t=2 .0 33~ 3.15 0 ,P<0 .0 1)。药物干预组视网膜光凝术眼眼压和各象限前房角均无显著改变。药物干预组睫状体脱离、前段脉络膜脱离的发生率分别为 11.11%、5 .5 6 % ,对照组分别为86 .11%、88.89%。药物干预组睫状体脉络膜脱离的高度亦显著低于对照组。 结论 达纳康能在一定程度上减少视网膜光凝术所致的睫状体脉络膜脱离和眼压升高、前房角变窄等眼前段并发症的发生率 ,减轻睫状体脉络膜脱离的程度。
Objective To investigate the intervention effect of Danakang on the anterior segment of retina after diabetic retinopathy (DR). Methods 72 eyes of 72 patients with DR treated with retinal photocoagulation (DR - stage) were divided into two groups: the intervention group and the control group by prospective randomized controlled trial. Ultrasound biomicroscopy (UBM) was used to observe and quantitatively analyze the changes of anterior chamber depth, anterior chamber angle, ciliary body and anterior choroid at 3 and 7 days after retinal photocoagulation in both groups. Results In the control group, the intraocular pressure of retinal photocoagulation was increased on the 3rd day after operation, which was significantly different from that before operation (t = -2.282, P <0.01). In addition to the nasal anterior chamber The anterior chamber angle of the rest of the quadrant outside the angle was narrower than that before the operation (t = 2.033 ~ 3.150, P <0.01). There was no significant change in intraocular pressure and the anterior chamber angle of each quadrant in the drug intervention group. The incidence of ciliary body detachment and anterior choroidal detachment in the intervention group was 11.11% and 5.56% respectively, while that in the control group was 86.11% and 88.89% respectively. The height of ciliary choroidal detachment in the drug intervention group was also significantly lower than that in the control group. Conclusions Danacone can reduce the incidence of anterior segment complications such as ciliary choroidal detachment, elevated intraocular pressure and narrowing of the anterior chamber angle caused by retinal photocoagulation to a certain extent, and reduce the degree of choroidal detachment .
