目的:应用阴离子间隙(AG)值判断混合型酸碱平衡紊乱。方法:抽取动脉血2ml均以肝素抗凝即时送检。以公式AG=[Na+]-[HCO3-]-[CL-]计算AG值,参考范围8~16mmol/L。酸碱血气分析结果按预计代偿公式计算,并结合AG值及临床诊断、病史、用药等情况,做出酸碱平衡紊乱类型的诊断。在此基础上以AG>16mmol/L为筛选标准,筛选出63例;63例又分为2组:单纯型酸碱平衡紊乱7例和混合型酸碱平衡紊乱56例。全部数据以珚x±s表示,组间比较采用方差分析。结果:混合型酸碱平衡紊乱AG值升高的例数明显多于单纯型酸碱平衡紊乱AG值升高的例数(P<0.05)。结论:因此,AG值升高,可帮助诊断混合型酸碱平衡紊乱,特别是对伴有代谢性酸中毒的混合型酸碱平衡紊乱的诊断具有重要意义。
Objective: To determine the mixed acid-base balance disorder using the value of anion gap (AG). Methods: 2ml arterial blood were heparin anticoagulant immediately sentinel. The AG value is calculated by the formula AG = [Na +] - [HCO3 -] - [CL-], and the reference range is 8-16 mmol / L. Acid-base blood gas analysis results calculated according to the expected compensation formula, combined with the AG value and clinical diagnosis, history, medication, etc., to make the diagnosis of acid-base balance disorders type. On this basis, 63 cases were screened with AG> 16mmol / L as screening criteria; 63 cases were divided into two groups: 7 cases with pure type acid-base balance disorder and 56 cases with mixed acid-base balance disorder. All data to 珚 x ± s, said the analysis of variance between groups. Results: The number of elevated AG values in mixed acid-base disorders was significantly higher than that in simple acid-base disorders (P <0.05). CONCLUSIONS: An elevated AG value may therefore help to diagnose mixed acid-base balance disorders, especially for the diagnosis of mixed acid-base balance disorders associated with metabolic acidosis.
