目的:探讨乌司他丁对五步蛇咬伤中毒后氧化应激及炎症反应的影响。方法:98例五步蛇咬伤患者随机分成对照组(n=48)和观察组(n=50);对照组给予抗蛇毒血清及支持对症治疗,观察组在对照组治疗基础上加用乌司他丁,治疗5d,分别于患者入院24小时内、给药后1d、给药后3d及给药后5d检测血清超氧化物歧化酶(SOD)、丙二醛(MDA)、肿瘤坏死因子-α(TNF-α)及白细胞介素6(IL-6)的水平。结果:与治疗前相比,患者在治疗后各时间点TNF-α、IL-6及MDA水平均显著降低(P<0.05),但观察组下降更明显(P<0.01);SOD水平均升高(P<0.05),但观察组升高更明显(P<0.01);与对照组相比,治疗后观察组各时间点TNF-α、IL-6及MDA水平均降低(P<0.05),SOD水平均升高(P<0.05)。结论:乌司他丁可降低五步蛇咬伤患者血清TNF-α、IL-6及MDA水平,升高SOD水平,减轻氧化应激及抑制炎症反应,因此有利于减轻五步蛇咬伤所致中毒的损害。
Objective: To investigate the effect of ulinastatin on oxidative stress and inflammatory response after five step snake bite poisoning. Methods: Ninety-eight patients were divided into control group (n = 48) and observation group (n = 50) randomly. The control group was given anti-snake venom serum and support symptomatic treatment. Stinging for 5 days, the levels of serum superoxide dismutase (SOD), malondialdehyde (MDA), tumor necrosis factor -α (TNF-α) and interleukin-6 (IL-6) levels. Results: Compared with those before treatment, the levels of TNF-α, IL-6 and MDA decreased significantly (P <0.05), but decreased more significantly in the observation group (P <0.01); the level of SOD increased The levels of TNF-α, IL-6 and MDA in the observation group were significantly lower than those in the control group (P <0.05) , SOD levels were increased (P <0.05). Conclusion: Ulinastatin can reduce the levels of serum TNF-α, IL-6 and MDA in patients with five-step snake bite, increase the level of SOD, reduce the oxidative stress and inhibit the inflammatory reaction, Toxin poisoning damage.
