目的 探讨甲状腺功能亢迸症(简称甲亢)患者注意偏侧化变化及其机制.方法 选取甲亢患者23例(甲亢组),体检健康者21例(对照组).采用简易精神状态量表(MMSE)评估精神状态;采用数字广度正序(DSF)、数字广度倒序(DSB)评估注意力、工作记忆及执行功能;采用汉密尔顿焦虑(HAMA)及抑郁(HAMD)量表评估焦虑和抑郁状态.采用偏侧化注意网络试验(LANT)程序检测双侧视野对不同箭头类型和线索类型的反应时间(RT),通过不同条件下的RT计算注意网络效率.采用Pearson相关分析法分析视野定向网络与神经心理学指标、甲状腺激素水平的关系.结果 甲亢组DSF低于对照组,HAMD、HAMA评分高于对照组,组间比较P均0.05).两组左、右侧视野不同线索类型RT比较差异均有统计学意义(P均RT双线索 >RT有效线索;左、右侧视野不同箭头类型下RT比较差异均有统计学意义(P均RT方向一致.甲亢组左、右侧视野的定向网络效率、警觉网络效率和执行控制网络效率比较差异均无统计学意义(P均>0.05).对照组右侧视野的定向网络效率高于左侧视野(P0.05).甲亢组右侧视野定向网络效率与血清FT3水平呈负相关(r=-0.463,P=0.046).结论 甲亢患者左侧半球偏侧化优势消失,可能与其甲状腺激素水平异常有关.“,”Objective To investigate the changes of the laterality of attention networks in patients with hyperthyroid -ism and its mechanism.Methods Twenty-three hyperthyroid patients(hyperthyroid group)and 21 health controls(con-trol group)were selected.Digit span forward(DSF)and digit span backward(DSB)were performed to assess the atten-tion,working memory and executive function.The Hamilton anxiety(HAMA)scale and Hamilton depression(HAMD) scale were performed to evaluate the anxiety and depression situation.The lateralized attention network test(LANT)was used to detect reaction time(RT)of both visual fields in different target types and cue types;according to RTs in different condition,we assessed the efficiency of the network.Pearson correlation analysis was used to analyze the relationship be-tween neuropsychological scales,thyroid hormone level, and the orienting network in visual field.Results DSF scores were lower,HAMD and HAMA scores were higher in the hyperthyroid group as compared with those of the control group(P0.05). There was significant difference in RT under different target types in both visual fields between the two groups(all PRT double cue>RT valid cue.There was significant difference in RT under different cue types,and the order was RT incongruent >RT congruent(all P0.05).Orienting efficiency of the right visual field was higher than that of the left visual field in the control group(P0.05).The orienting efficiency of the right visual field was negatively correlated with the level of FT3(r=-0.463, P=0.046).Conclusion The advantage of left hemisphere lateralization disappears, which may be related to abnormal thyroid hormone level.
