目的:探讨血浆外泌体环状RNA(circRNA)hsa_circ_0022417在胃癌中的表达及其临床意义。方法:选取临床明确诊断胃癌初诊患者60例,慢性胃炎患者30例(疾病对照组)和同期健康体检者30名(健康对照组),分别检测各组血浆外泌体中hsa_circ_0022417的表达水平以及血清CEA、CA19-9的表达。采用ROC曲线评价各项指标诊断胃癌效能。分析血浆外泌体hsa_circ_0022417的表达与胃癌患者临床病理参数的关系。结果:胃癌组血浆外泌体hsa_circ_0022417的相对表达水平高于慢性胃炎组和健康对照组,差异有统计学意义[(1.89±0.15)比(0.93±0.08)比(0.75±0.07),n F=9.96,n P<0.05]。胃癌组织中hsa_circ_0022417的表达水平高于癌旁组织,差异有统计学意义[(2.78±0.24)比(1.18±0.11),n t=6.08,n P<0.05],并且其表达水平与血浆外泌体表达水平呈正相关(n r=0.72,n P<0.05)。ROC曲线分析结果显示,血浆外泌体hsa_circ_0022417、血清CEA、CA19-9单指标诊断胃癌的曲线下面积分别为0.79、0.68和0.66,3种指标联合检测诊断胃癌的曲线下面积为0.86。血浆外泌体hsa_circ_0022417的相对表达量与肿瘤大小(n χ2=6.42,n P<0.05)、分化程度(n χ2=5.83,n P<0.05)、临床分期(n χ2=7.14,n P<0.01)及淋巴结转移(n χ2=5.17,n P<0.05)均有关。n 结论:外泌体hsa_circ_0022417在胃癌患者血浆中高表达,对临床胃癌辅助诊断和早期筛查具有重要意义。“,”Objective:To investigate expressions and clinical significance of plasma exosomal hsa_circ_0022417 in gastric cancer (GC).Methods:Sixty gastric cancer patients, 30 chronic gastritis patients (disease control group) and 30 healthy volunteers (healthy control group) were enrolled in this study. The expression levels of plasma exosomal hsa_circ_0022417 and serum CEA and CA19-9 were detected. The ROC curve and AUC were used to estimate the diagnostic capacity.Results:Compared with chronic gastritis patients and healthy control, the expression of plasma exosomal hsa_circ_0022417 was significantly upregulated in the gastric cancer group (n F=9.96, n P<0.05). The expression level of hsa_circ_0022417 in GC tissue was significantly higher than that in adjacent tissue (n t=6.08, n P<0.05). The AUC of hsa_circ_0022417, serum CEA and CA 19-9 was 0.79, 0.68 and 0.66, respectively. The combined detection of three indicators had the highest AUC (0.86) (n P<0.05). The expression level of exosomal hsa_circ_0022417 was significantly correlated with tumor size (n χ2=6.42,n P<0.01), differentiation degree (n χ2=5.83,n P=0.05), TNM stage (n χ2=7.14,n P<0.05) and lymph node metastasis (n χ2=5.17,n P<0.05).n Conclusion:Exosome hsa_circ_0022417 is highly expressed in the plasma of GC patients, which is of great significance for clinical auxiliary diagnosis and early screening of GC.
