目的:观察不同解剖类型的上颌第一磨牙在不同力值作用下倾斜角度的变化,初步探讨上颌第一磨牙与周围组织解剖结构关系在正畸研究中的意义。方法:应用多排螺旋CT对62例患者正畸前上颌骨进行扫描并三维重建,依据上颌第一磨牙与上颌窦的解剖关系分为嵌顿组和非嵌顿组。根据CT图像测量上颌第一磨牙长轴与正中轴的角度,所有病例正畸过程中对上颌第一磨牙施加恒定力值,正畸治疗结束后再次对患者上颌骨进行扫描重建及角度测量,计算前、后角度之差即为倾斜角度,统计软件分析两组数据倾斜角度的关系。结果:嵌顿组倾斜角度明显大于非嵌顿组。结论:临床上上颌第一磨牙矫正施力设计可根据多排螺旋CT的扫描结果作为参考,嵌顿型加力或力值调整应比非嵌顿型保守。
OBJECTIVE: To observe the changes of tilt angle of maxillary first molar with different anatomical types under different force values, and to explore the significance of the relationship between the anatomic structure of the maxillary first molar and the surrounding tissue in orthodontic study. Methods: Sixty-two patients underwent maxillary reconstruction and three-dimensional reconstruction with multi-slice spiral CT. According to the anatomic relationship between maxillary first molar and maxillary sinus, they were divided into incarcerated group and non-incarcerated group. According to the CT images, the angle between the long axis of the maxillary first molar and the median axis was measured. During the orthodontics, a constant force was applied to the maxillary first molar. After the orthodontic treatment, the maxillary reconstruction was performed and the angle was measured again The difference between the front and back angles is the tilt angle, and the statistical software analyzes the relationship between the tilt angles of the two sets of data. Results: The incline angle of incarcerated group was obviously larger than that of non-incarcerated group. Conclusion: The maxillary first molar orthodontic force design can be based on the multi-slice spiral CT scan results as a reference, incarcerated force or force adjustment should be conservative than the non-incarcerated type.
