目的探讨GATA-3蛋白与膀胱尿路上皮癌的临床生物学特征、术后复发和P53、Ki-67蛋白表达的关系。方法收集膀胱尿路上皮癌组织85例,采用免疫组化二步法检测组织GATA-3、P53和Ki-67蛋白的表达,并分析GATA-3阳性表达与患者年龄、性别、临床分期、肿瘤分级、肿瘤大小和肿瘤数目等的关系,并根据术后复发风险评估标准分为低危组、中危组和高危组,评估GATA-3蛋白与术后复发的关系。结果 GATA-3蛋白的阳性表达率为55.29%,在GATA-3蛋白阳性组中,P53和ki-67的阳性表达率为78.72%和17.02%,在GATA-3蛋白阴性组中,P53和ki-67的阳性表达率为42.11%和84.21%,差异具有统计学意义(P<0.05),相关分析显示GATA-3与P53和ki-67的表达呈相关(r=0.874和-0.746,P=0.001)。GATA-3的表达与肿瘤临床分期、肿瘤分级和肿瘤大小密切相关(P<0.05),而与患者年龄、性别、淋巴结转移和肿瘤数目等无关(P>0.05)。在不同术后复发风险组中,中危组GATA-3的表达阳性率明显高于高危组,组间比较差异具有统计学意义(P<0.05)。结论 GATA-3参与调控膀胱尿路上皮癌的恶性生物学行为,其高表达常常提示患者术后具有较低的复发率和较高的生存质量,具有重要的指导意义。
Objective To investigate the clinical biological features of GATA-3 protein and bladder urothelial carcinoma, the relapse after surgery and the relationship between the expression of P53 and Ki-67. Methods Totally 85 bladder urothelial carcinoma tissues were collected. The expressions of GATA-3, P53 and Ki-67 were detected by immunohistochemical two-step method. The positive expression of GATA-3 was analyzed with age, gender, clinical stage, Grade, tumor size and tumor number. According to the criteria of postoperative recurrence risk, the patients were divided into low-risk group, moderate-risk group and high-risk group to evaluate the relationship between GATA-3 protein and postoperative recurrence. Results The positive rate of GATA-3 protein was 55.29%. The positive rates of P53 and ki-67 in GATA-3 protein positive group were 78.72% and 17.02% respectively. In GATA-3 negative group, P53 and ki The positive rate of GATA-3 was 42.11% and 84.21%, respectively, with statistical significance (P <0.05). Correlation analysis showed that the expression of GATA-3 was correlated with the expression of P53 and Ki67 (r = 0.874 and -0.746, P = 0.001). The expression of GATA-3 was closely related to clinical stage, tumor grade and tumor size (P <0.05), but not to age, gender, lymph node metastasis and tumor number (P> 0.05). The expression of GATA-3 in the intermediate risk group was significantly higher than that in the high risk group (P <0.05). Conclusions GATA-3 is involved in the regulation of malignant biological behavior of bladder urothelial carcinoma. The high expression of GATA-3 often suggests that patients have a low recurrence rate and high quality of life after the operation, which is of guiding significance.
