目的:探索抗炎治疗对白细胞精液症患者精子参数的改善情况及对精浆瘦素(Leptin)与活性氧(Reactive oxygen species, ROS)水平的影响,并分析其相关性。方法随机选取2010年12月至2012年6月上海交通大学医学院附属仁济医院泌尿男科门诊的白细胞精液症患者80例(WBC组),另选取各项参数均正常的40份精液标本作为对照组,在患者治疗前及经治疗精液白细胞水平恢复正常后,采用计算机辅助精液分析系统(CASA)行精液常规检测、放射免疫法检测精浆Leptin、TBA法检测精浆ROS。比较各参数的组间差异并分析其相关性。结果WBC组治疗前精子浓度及活力均低于对照组(P<0.05,P<0.01),精浆Leptin与ROS水平高于对照组(P<0.05, P<0.01),精浆Leptin水平与精浆ROS水平具有正相关性(r =0.695, P <0.05),经治疗后精子活力显著增加(P<0.01),精浆Leptin及ROS水平亦明显下降(P<0.05,P<0.01)。结论白细胞精液症患者精浆Leptin、ROS水平均有增加,精子易受到氧化损伤导致其密度、活力下降,精浆Leptin与ROS水平之间存在正相关性,提示Leptin可能参与了氧化应激反应导致精子浓度和活力的下降。“,”Objective To investigate the levels of seminal Leptin, ROS, as well as sperm parameters in leukocytospermia patients tretaed with anti-inflammatory therapyand explore their correlation. Methods Total of 80 male infertility patients with leukocytospermia (WBC group) were enrolled in the study, and 40 normal sperm samples were taken as the control. The sperm parameters were detected by computer assisted sperm analysis (CASA). The levels of semen plasma Leptin and ROS were measured using RIA and TBA kits respectively. Results The sperm concentration and viability in WBC group before treatment were lower than those of the control group (P<0.05,P<0.01), whereas, the levels of semen plasma Leptin and ROS were higher than those of the control group (P<0.05, P<0.01). The level of Leptin in semen plasma was positively correlated with the ROS level (r=0.695, P<0.05). After treatment, the increase of semen viability and decrease of Leptin and ROS level were found in WBC group. Conclusion Increased levels of seminal Leptin and ROS were founded in WBC group. Positive relationship between seminal Leptin and ROS indicated that Leptin might involve in oxidative stress and result in the decrease of sperm concentration and viability.
