Nearly known to the brain capillary endothelial cells is the anatomy of the blood-brain barrier, and the role of this endothelial cell is to maintain the brain’s interstitial fluid composition and volume constant. Defects in brain capillary function seem to play a role in the pathogenesis of cerebral edema and hemorrhage in a variety of diseases. Intracerebral hemorrhage in immature infants, neonatal asphyxia, lead poisoning, Reye’s syndrome, penetrating coma, and brain edema around brain tumors or brain abscess, all have common features of cerebral capillary failure. This article presents several new developments we know about cerebral capillaries to explain why it is particularly vulnerable.
