目的:评价CAD/CAM导板在上前牙区即刻种植的临床效果及其美学意义。方法:选取我院30例因外伤等原因需要行即刻种植的患者,将CBCT图像和石膏模型扫描数据配准,合成三维颌骨模型,通过Simplant软件设计种植体植入位置,制作CAD/CAM种植牙导板。在导板的引导下进行种植窝的预备,并植入种植体,术后拍摄CBCT。结果:应用Mimics软件对比术后与术前设计的种植体三维位置,30例患者共46枚种植体的三维位置与术前设计基本一致。讨论:种植体准确的三维位置是获得美学修复效果的绝对必要条件,CAD/CAM导板是确定种植位点的有效手段。然而,目前的导板制作流程复杂、费用较高,尚难以在临床上常规应用。结论:CAD/CAM导板对美学区即刻种植的定位效果良好,但仍需要进一步改进,以满足临床需要。
OBJECTIVE: To evaluate the clinical effect and aesthetic significance of CAD / CAM guide plate in immediate anterior extraction. Methods: Thirty patients in our hospital who needed immediate implantation due to trauma were enrolled in the study. The CBCT images and the gypsum model scan data were registered to synthesize the three-dimensional jaw model. The implantation position of the implant was designed by Simplant software to make CAD / CAM implant Tooth guide plate. Under the guidance of the guide plate for planting nest preparation, and implantation of implants, postoperative CBCT. Results: The three-dimensional position of the implant after surgery and preoperative design was compared with Mimics software. The three-dimensional position of 46 implants in 30 patients was basically the same as the preoperative design. Discussion: The accurate three-dimensional position of the implant is an absolutely necessary condition to obtain aesthetic restoration effect. The CAD / CAM guide plate is an effective means to determine the planting site. However, the current guide plate making process is complicated and costly, which is still difficult to be routinely used clinically. Conclusion: The CAD / CAM guide plate has a good effect on the immediate planting of aesthetics area, but it needs further improvement to meet the clinical needs.
