目的:探讨软性输尿管镜钬激光碎石术(FURL)治疗单侧上尿路结石的疗效及对外周血T淋巴细胞亚群(血清CD3n ＋、CD4n ＋、CD8n ＋、CD4n ＋/CD8n ＋)、肾功能[血尿素氮(BUN)、血肌酐(Scr)、β2-微球蛋白(β2-MG)]及氧化应激因子[血红素氧合酶-1(HO-1)、血浆内皮素-1(ET-1)、皮质醇(COR)及促血管生成素-Ⅱ(Ang-Ⅱ)]的影响。n 方法:选择本院泌尿外科2017年8月至2019年5月收治的上尿路结石患者100例，采用随机数字表法分为对照组和观察组，各50例。对照组采用经皮肾镜取石术(PCNL)治疗，观察组采用FURL治疗。比较两组手术时间、住院时间、结石清除率、术后24 h肾功能、氧化应激因子及T淋巴细胞亚群的变化情况。结果:观察组术后住院时间少于对照组(n P<0.05)，手术时间及结石清除率均高于对照组(n P0.05)；术后24 h，观察组BUN、Scr、β2-MG、HO-1、ET-1、COR、Ang-Ⅱ等指标均低于对照组(n P<0.05)，外周血CD3n ＋、CD4n ＋、CD8n ＋、CD4n ＋/CD8n ＋均高于对照组(n P<0.05)，CD8n ＋低于对照组(n P<0.05)；观察组手术并发症总发生率6.00%(3/50)低于对照组22.00%(11/50)(n P<0.05)。n 结论:FURL对单侧上尿路结石患者的取石成功率高，术后并发症少，恢复快，住院时间短，氧化应激反应低，对外周血细胞免疫功能的抑制作用小，临床疗效显著，安全性高，值得临床推广和应用。“,”Objective:To investigate the effect of flexible ureteroscopic holmium laser lithotripsy (FURL) on unilateral upper urinary calculi and its influence on T lymphocyte subsets, renal function and oxidative stress factors in peripheral blood.Methods:From August 2017 to May 2019, 100 patients with upper urinary tract stones admitted to our urology department were selected.Random number table method was used to divide the patients into control group and observation group, 50 cases with each group.The control group was treated with percutaneous nephrolithotomy, while the observation group was treated with flexible ureteroscopic holmium laser lithotripsy.The operation time, hospitaliza-tion time, stone clearance rate, renal function, oxidative stress factor and T lymphocyte subsets were compared between the two groups 24 hours after operation.Results:The hospitalization time of the observation group was less than that of the control group, and the operation time and stone clearance rate were longer than or higher than those of the control group(n P0.05). Twenty-four hours after operation, BUN, Scr, β2-MG, HO-1, ET-1, COR and Ang-II in the observation group increased less than those in the control group, while, CD3n +、CD4n +、CD8n +、CD4n +/CD8n + levels increased, and CD8n + levels decreased more than those in the control group(n P<0.05). The total incidence of operative complications in the observation group was 6.00% lower than that in the control group 22.00%(n P<0.05).n Conclusions:FURL has the advantages of high success rate, fewer complications, quick recovery, short hospitalization time, low oxidative stress response, low inhibitory effect on peripheral blood cell immune function, remarkable clinical efficacy and high safety.It is worthy of clinical promotion and application.
