保留前列腺被膜膀胱全切除后行乙状结肠代膀胱吻合术的体会

膀胱癌膀胱全切除后,一般应用游离肠段施行各种尿流改道手术,如回肠膀胱术,直肠膀胱乙状结肠腹壁造瘘术等。这些手术因改变了尿道的排尿生理状态,给病人带来不便。本文自1984~1988年1月对5例膀胱癌患者施行了保留前列腺被膜膀胱全切乙状结肠代膀胱术,克服了上述不足。1 资料与方法1.1 临床资料 全组均为男性。年龄44~62岁。按膀胱癌移行上皮细胞分化程度分类:Ⅲ级3例,Ⅱ级2例。肿瘤组织侵及膀胱肌层或浆肌层。术前排除乙状结肠病变,并按一般结肠肿瘤术前准备进行。 Bladder cancer bladder resection, the general application of free intestinal segment of the implementation of a variety of urinary diversion surgery, such as ileal bladder surgery, rectum, bladder sigmoid abdominal fistula surgery. Age 44 ~ 62 years old. According to the classification of bladder transitional epithelial cell differentiation: 3 cases in grade Ⅲ and 2 cases in grade Ⅱ. Tumor invasion of the bladder muscle or muscle layer. Preoperative sigmoid colon lesions excluded, and according to the general colon tumor preoperative preparation.
