目的点评我院门诊处方并对其质量进行分析。方法抽取2014年1至6月医院各科室门诊处方3000张,点评抽取处方并结合点评结果分析处方质量。结果门诊各科室3000张处方中,合格处方2535张,合格率84.5%;不合格处方465张,按处方点评可归类为超常处方3.0%(14/465)、不规范处方26.7%(124/465)以及用药不合理处方70.3%(327/465),其中用药不合理处方比例明显高于超常处方和不规范处方,差异有统计学意义(P<0.05)。结论医院应在处方点评并分析处方质量的基础上,进一步加强处方管理;医师应严格规范开具处方;临床药师则应严格把关,全面监督并审核处方,保障合理用药与医疗安全。
The purpose of our hospital outpatient prescriptions and their quality analysis. Methods From January to June 2014, 3000 outpatient prescriptions were collected from all departments of the hospital. The prescriptions were extracted from the reviews and the quality of the prescriptions was analyzed in conjunction with the review results. Results Out of the 3000 prescriptions in outpatient departments, 2535 were qualified, with a pass rate of 84.5%; 465 unqualified prescriptions, which were classified as supernormal prescriptions by 3.0% (14/465) and non-standard prescriptions (26.7% 465) and 70.3% (327/465) of unreasonable prescriptions. The proportion of unreasonable prescriptions was significantly higher than that of over-prescription and non-standard prescriptions, the difference was statistically significant (P <0.05). Conclusions The hospital should further improve the prescription management based on the prescription reviews and the analysis of prescription quality. The physicians should strictly regulate the prescription. The clinical pharmacists should strictly monitor and supervise and review prescriptions to ensure the rational use of medicines and medical safety.
