目的探讨声导抗及听性脑干反应(ABR)测试在成人分泌性中耳炎(SOM)诊断中的意义及ABR测试Ⅰ、Ⅲ、Ⅴ波潜伏期与纯音听阈测听气骨导差之间的关系。方法采用丹麦奥迪康AZ型声导抗仪对29例(44耳)SOM成人进行声导抗测试,记录其鼓室压、声导纳值、容积等,并与健康组进行对比分析;ABR测试,其中单耳SOM14例,双耳SOM15例,记录其Ⅰ、Ⅲ、Ⅴ波潜伏期,纯音听阈测试记录其气骨导差,将结果分别与Ⅰ、Ⅲ、Ⅴ波潜伏期进行相关分析。结果SOM成人中44耳声导纳为(0.17±0.13)mL,与健康组比较差异有统计学意义(P<0.01);ABR测试44耳Ⅰ、Ⅲ、Ⅴ波潜伏期与健康组差异有统计学意义(P<0.01),明显延长,与气骨导差平均值进行直线相关分析,二者呈高度正相关,相关系数分别为0.965、0.899、0.913;结论声导抗测试在诊断成人SOM中具有十分重要的意义,ABR测试起辅助作用,Ⅰ、Ⅲ、Ⅴ波潜伏期明显延长,且随着气骨导差的增大而延长。
Objective To investigate the significance of acoustic impedance and auditory brainstem response (ABR) testing in the diagnosis of adult secretory otitis media (SOM) and the relationship between the latency of Ⅰ, Ⅲ and Ⅴ wave and ABR test . Methods Twenty-four (44-ears) SOM adults were tested for acoustic impedance using the Danish Audiicon AZ-type acoustic impedance meter. The tympanic pressure, acoustic admittance and volume were recorded and compared with those of the healthy group. ABR test, Among them, 14 cases of single-ear SOM and 15 cases of double-ear SOM recorded the latency of Ⅰ, Ⅲ and Ⅴ wave, pure tone audiometry test recorded the air-conduction difference, and the results were correlated with latency of Ⅰ, Ⅲ and Ⅴ respectively. Results 44 ears acoustic admittance was (0.17 ± 0.13) mL in SOM adults, which was significantly different from that in healthy subjects (P <0.01). There was statistical difference between Ⅰ, Ⅲ and Ⅴ waves in ABR test (P <0.01), significantly prolonged, and linear correlation analysis with the average value of air conduction, the two were highly positive correlation, the correlation coefficients were 0.965,0.899,0.913; Conclusion Acoustic impedance test in the diagnosis of adult SOM has Very important meaning, ABR test plays a supporting role, Ⅰ, Ⅲ, Ⅴ wave latency was significantly prolonged, and with the increase of air conduction increased.
