目的:探讨采用RevoLix 2μm激光手术系统治疗BPH对患者性功能的影响。方法:对120例BPH患者进行了RevoLix 2μm激光手术治疗,全部病例均行IPSS评分。利用国际勃起功能障碍问卷-5(IIEF-5)和自制统一的射精障碍调查表,分别于术前、术后半年了解患者阴茎勃起功能情况、性欲、射精情况及对性生活满意程度进行调查。结果:全部手术均成功,IPSS有明显改善,从(25.12±3.21)分下降为(6.26±2.12)分,(P<0.05)。术后患者均保留性功能。IIEF-5术前评分为(23.50±3.12)分,术后半年为(22.40±4.62)分;术前有性欲者占81.67%,术后半年占85.00%,手术前后比较差异无统计学意义(P>0.05)。术前射精异常者占23.33%,术后半年射精异常者占71.67%,术后射精功能较术前明显下降(P<0.05)。术前对性生活满意者占65.00%,术后占70.00%,手术前后比较差异无统计学意义(P>0.05)。结论:RevoLix 2μm激光手术系统治疗BPH有效安全,术中、术后并发症少,对患者阴茎勃起功能、对性生活满意程度及性欲无明显影响,其影响主要为射精障碍。术中操作仔细,防止损伤影响阴茎勃起的神经血管束,术后处理得当,可减少或避免对患者的性功能影响。
Objective: To investigate the effect of RevoLix 2μm laser surgery system on the treatment of patients with sexual function. Methods: 120 cases of BPH patients were treated with RevoLix 2μm laser surgery, all patients underwent IPSS score. The questionnaire of erectile dysfunction -5 (IIEF-5) and self-made questionnaire of ejaculation disorder were used to investigate the erectile dysfunction, libido, ejaculation and satisfaction with sex life before and 6 months after operation. Results: All the operations were successful and the IPSS was significantly improved from (25.12 ± 3.21) points to (6.26 ± 2.12) points (P <0.05). Postoperative patients retain sexual function. The preoperative score of IIEF-5 was (23.50 ± 3.12) points and was (22.40 ± 4.62) six months after operation. The preoperative libido was 81.67% and the postoperative half-year was 85.00%. There was no significant difference before and after operation P> 0.05). Preoperative ejaculation accounted for 23.33% of abnormalities, six months after ejaculation abnormalities accounted for 71.67%, postoperative ejaculation function was significantly decreased compared with preoperative (P <0.05). Preoperative satisfaction with sexual life accounted for 65.00%, postoperative 70.00%, no significant difference between before and after surgery (P> 0.05). Conclusion: The RevoLix 2μm laser surgery system is effective and safe for the treatment of BPH. There are few intraoperative and postoperative complications, which have no significant effect on erectile function, sexual satisfaction and libido. Careful operation during surgery to prevent damage to the penis erection of the neurovascular bundle, postoperative treatment properly, can reduce or avoid the impact of sexual function in patients.
