目的：观察用硬化剂注入翼外肌下头治疗颞颌关节复发性脱位（ＲＴＭＪＤ）效果。方法：以标有长度的７号注射针头于髁突颈稍前方平行髁突横轴进行深达２．０ｃｍ～２．２ｃｍ，缓慢后退１．０ｃｍ～１．２ｃｍ，并同时注入０．６ｍｌ～０．８ｍｌ硬化剂后拔针。结果：经治疗的２５例３２例ＲＴＭＪＤ平均随访３年，治愈率达９４％；ＴＭＪ许勒位片显示治疗后的髁状突前伸范围明显小于治疗前（Ｐ＜０．０１），平均缩短４ｍｍ左右。结论：该方法治疗ＲＴＭＪＤ简单方便，疗效可靠。其机制可能是硬化剂破坏了部分肌纤维，削弱了大开口时髁突所受的前拉力，缩小了髁突的前伸运动范围而达到治疗目的。
Objective: To observe the effect of infusing the extramedullary muscle with hardener on the treatment of temporomandibular joint recurrent dislocation (RTMJD). Methods: The length of the 7th injection needle in the condylar neck slightly parallel to the horizontal axis of the condyle before the depth of 2.0cm ~ 2.2cm, slowly back 1.0cm ~ 1.2cm, and while injecting 0.6ml ~ 0.8ml hardener pull the needle. Results: The 32 cases of RTMJD treated in 25 cases were followed up for an average of 3 years and the cure rate was 94%. The TMJ radiolucer film showed that the range of condyle prolongation was obviously shorter than before treatment (P <0.01) 4mm or so. Conclusion: This method is simple and convenient to treat RTMJD and has reliable curative effect. The mechanism may be that the hardener destroys part of the muscle fiber, weakens the pre-tension of the condylar process when the large opening is opened, and narrows the reach of the condyle to reach the therapeutic purpose.
