目的 探讨累积粉尘暴露量对左室舒张功能的影响.方法 选择在开滦职业病防治院做康复治疗的789例尘肺病患者进行血生化及心脏超声检查,根据累积粉尘暴露量(cumulative dust exposure,CDE)将研究对象按照四分位数分组,应用SPSS 17.0软件进行统计分析.结果 偏相关分析显示,CDE与二尖瓣EF斜率呈负相关(P<0.05);线性回归分析显示,校正其他混杂因素后,CDE、年龄、收缩压(SBP)、体质量指数(BMI)与二尖瓣EF斜率呈负线性相关(P<0.05);影响二尖瓣EF斜率减低的多因素logistic回归分析显示,校正了年龄、心率(HR)、BMI、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL)、高血压病、糖尿病、吸烟等因素后,与第1分位组比较,第4分位组二尖瓣EF斜率<80 mm/s的检出率增加,其OR值为2.04(95% CI为1.23~3.37).结论 CDE是左室舒张功能下降的危险因素.“,”Objective To investigate the effects of CDE on left ventricular diastolic function. Methods 789 cases of pneumoconiosis patients treated by rehabilitation in our hospital were selected to carry out blood biochemistry and cardiac ultrasound examination. SPSS17.0 was used to statistically analyse the examination result. Results Partial correlation analysis showed that there was a negative correlation between CDE and the Mitral EF slope(P<0.05). Linear regression analysis showed that after adjusting for other confounding factors,CDE,age,SBP,and BMI were negatively correlated with the mitral valve EF slope(P<0.05). Multiple logistic regression analysis showed that after adjusting for age,HR,BMI,TC,TG,LDL,hypertension,diabetes and smoking,the detection rate of mitral valve EF<80mm/s was increased in quartile 4 compared to quartile 1,and OR values was 2.04(95% CI1.23~3.37). Conclusions CDE is a risk factor for the decrease of left ventricular diastolic function.
