目的比较胰岛素泵与每日多次注射胰岛素对初诊糖尿病患者的降糖效果。方法 2013年12月-2014年2月采用对照研究的方法,将36例初诊糖尿病患者随机分为胰岛素泵组(18例)和皮下注射组(18例),胰岛素泵组采用胰岛素泵持续皮下注射重组人胰岛素注射液,皮下注射组采用专用胰岛素笔三餐前皮下注射重组人胰岛素注射液+睡前皮下注射精蛋白锌重组人胰岛素注射液常规治疗。比较两组患者治疗1周后血糖控制水平、低血糖发生率、胰岛素用量及皮肤感染情况。结果治疗1周后两组患者8个时点的血糖水平比较差异均有统计学意义(P<0.001)。胰岛素泵组发生低血糖1例,皮下注射组发生低血糖6例,差异无统计学意义(P=0.088)。胰岛素泵组治疗1周后胰岛素用量(33.73±4.12)U,皮下注射组(43.61±9.18)U,差异有统计学意义(P<0.01)。胰岛素泵组无皮肤感染发生;皮下注射组发生皮下硬结1例,进针点发红3例。结论胰岛素泵模拟了正常人体胰腺胰岛素的分泌,可更快更有效地控制高血糖,减少并发症的发生,是糖尿病患者安全有效的选择。
Objective To compare the hypoglycemic effect of insulin pump with multiple daily injections of insulin on newly diagnosed diabetic patients. Methods From December 2013 to February 2014, 36 newly diagnosed diabetic patients were randomly divided into insulin pump group (18 cases) and subcutaneous injection group (18 cases). The insulin pump group was continuously subcutaneously injected with insulin pump Recombinant human insulin injection, subcutaneous injection group using special insulin pen three meals before subcutaneous injection of recombinant human insulin + bedtime injection of protamine zinc recombinant human insulin routine treatment. The level of glycemic control, incidence of hypoglycemia, insulin dosage and skin infection were compared between the two groups after 1 week of treatment. Results After 1 week of treatment, the blood sugar levels of the two groups at 8 time points were significantly different (P <0.001). 1 case of hypoglycemia in insulin pump group and 6 cases of hypoglycemia in subcutaneous injection group, the difference was not statistically significant (P = 0.088). Insulin pump group after 1 week of insulin dosage (33.73 ± 4.12) U, subcutaneous injection group (43.61 ± 9.18) U, the difference was statistically significant (P <0.01). No skin infection occurred in the insulin pump group; subcutaneous injection in 1 case of subcutaneous induration, red needle into the point in 3 cases. Conclusion Insulin pump simulates the secretion of insulin in normal human pancreas, which can control hyperglycemia more quickly and effectively and reduce the incidence of complications. It is a safe and effective choice for patients with diabetes mellitus.
