目的:运用二维超声心动图(2DE)、Tei指数、定量组织速度成像(QTVI)评价矽肺患者右室功能的改变。方法:对照组30例,矽肺组30例,所有研究对象行2DE右心常规超声指标测量;脉冲组织多普勒(PWTDI)模式下测算右室Tei指数;QTVI获取三尖瓣前叶及隔叶瓣环的心肌运动速度曲线,记录收缩期峰值运动速度(VS)、舒张早期运动速度(VE)、舒张晚期运动速度(VA),计算VE/VA。结果:与对照组相比,矽肺组右心大小及肺动脉压无明显变化,右室Tei指数明显增大(P<0.01);心肌VA增加、VE降低,VE/VA值降低(P<0.05或P<0.01)。结论:右室Tei指数及QTVI测量的右心室局部心肌组织运动速度可以早期、敏感、准确地反映矽肺患者右室功能的变化,是评价矽肺患者右室功能的较好指标。
Objective: To evaluate the change of right ventricular function in patients with silicosis by two-dimensional echocardiography (2DE), Tei index and quantitative tissue velocity imaging (QTVI). Methods: Thirty patients in the control group and 30 in the silicosis group were enrolled in the study. 2DE right ventricular conventional ultrasound was performed in all subjects. The right ventricular Tei index was calculated by pulsed tissue Doppler (PWTDI) Velocity curve of the annulus was recorded. The peak systolic velocity (VS), early diastolic velocity (VE) and late diastolic velocity (VA) were recorded to calculate VE / VA. Results: Compared with the control group, there was no significant change in right heart size and pulmonary arterial pressure in the silicosis group, and the Tei index in the right ventricle increased significantly (P <0.01 or P <0.01). The VA increased, the VE decreased and the VE / VA decreased P <0.01). CONCLUSION: Right ventricular Tei index and right ventricular myocardial velocity measured by QTVI can reflect the change of right ventricular function in patients with silicosis early and sensitively, which is a good indicator to evaluate the right ventricular function in patients with silicosis.
