目的 :探讨有晶状体眼前房型人工晶状体植入术矫正高度近视的有效性和安全性。方法 :高度近视眼患者 16例共 2 9只眼 ,近视度数 (- 7.0 0~ - 30 .0 0 ) D,行有晶状体眼前房型人工晶状体植入术。结果 :2 9只患眼均成功植入人工晶状体 ,术后随访 3~ 6个月。最佳矫正视力 :术前 (0 .5 0± 0 .2 6 )与术后 3个月 (0 .73± 0 .2 6 )差异无显著性 (t=2 .0 4 3,P=0 .0 5 1) ,但术后 3个月时最佳矫正视力均好于或等于术前 ;屈光状态 :术前为 (- 18.0 3± 5 .5 4 ) D,术后 3个月为 (- 0 .82± 1.5 4 ) D,差异有显著性 (t=30 .899,P=0 .0 0 ) ;平均眼压 :术前为 (2 .0 91± 0 .380 ) k Pa,术后3个月为 (1.734± 0 .5 72 ) k Pa,差异无显著性 (t=1.98,P=0 .0 7) ;平均角膜内皮细胞数目 :术前为 (2 70 4± 390 ) /mm2 ,术后 3个月为 (2 5 19± 2 78) / mm2 ,较术前减少 6 .84 % ,但差异无显著性 (t=1.16 ,P=0 .2 6 )。本组患者无严重并发症发生。结论 :有晶状体眼前房型人工晶状体植入术矫正高度近视 ,手术操作简单 ,具有较好的屈光状态的可预测性、可逆性和可控制性 ,但远期效果尚有待进一步观察。
Objective: To investigate the effectiveness and safety of phakic anterior chamber intraocular lens implantation for correction of high myopia. Methods: A total of 29 eyes of 16 patients with high myopia, myopia (- 7.0 0 ~ - 30. 0 0) D, anterior chamber phakic intraocular lens implantation. Results: All 29 eyes were successfully implanted intraocular lens, followed up for 3 to 6 months. The best corrected visual acuity was no significant difference between preoperative (0.05 ± 0.226) and 3 months (0. 73 ± 0.2 6) (t = 2.043, P = 0 .0531), but the best corrected visual acuity was better than or equal to preoperative at 3 months after operation. The refractive status was (-18.0 3 ± 5.54) D preoperatively and 3 months postoperatively (-0.82 ± 1.54) D, the difference was significant (t = 30.898, P = 0.000); mean intraocular pressure was (2 .091 ± 0.38) kPa before operation, The mean corneal endothelial cell number was (2 70 4 ± 390) preoperatively (1.734 ± 0.572) kPa at 3 months postoperatively, with no significant difference (t = 1.98, P = 0.07) / mm2, and (2 5 19 ± 2 78) / mm2 at 3 months after operation, which was 6 .84% lower than that before operation, but the difference was not significant (t = 1.16, P = 0.226). This group of patients without serious complications. CONCLUSION: The phakic anterior chamber intraocular lens implantation for correction of high myopia, surgical operation is simple, with good refractive predictability, reversibility and controllability, but the long-term effect remains to be further observed.
