本文采用经颅多普勒超声仪(TCD),检测3例尚未出现临床脑死亡指征的高血压脑血栓形成,脑梗塞致脑动脉血管灌注明显不足的患者,其中大脑中动脉(MCA)平均血流速度(Vm)6~16cm/s间,频谱呈舒张期血流信号消失,收缩期矮小峰信号,与一般的高血压、脑动脉硬化缺血性患者检测结果明显不同。3例患者均在近期内死亡。故我们认为,这是一种脑死亡前期的TCD表现,应用TCD检测有助于对缺血性脑死亡提出早期预报,可做为Kirkham诊断标准的补充。
In this study, transcranial Doppler ultrasound (TCD) was used to detect three cases of hypertensive cerebral thrombosis that did not show clinical signs of brain death, cerebral infarction caused significantly insufficient perfusion of cerebral arteries, including middle cerebral artery (MCA) mean blood Flow velocity (Vm) between 6 ~ 16cm / s, the spectrum was diastolic blood flow disappeared, shortening peak systolic signal, and the general hypertension, cerebral arteriosclerosis ischemic test results were significantly different. All three patients died in the near future. Therefore, in our opinion, this is a TCD manifestation of pre-brain death. Using TCD to detect early ischemic brain death can be used as an early supplement to Kirkham’s diagnostic criteria.
