比较容量控制反比通气(VC-IRV,简称IRV)和气道压力释放通气(APRV)模式对健康和急性肺损伤(ALI)犬心肺功能的影响。方法对健康和ALI模型(犬),分别用IRV和APRV模式进行通气,观察通气期间血流动力学、血气、呼吸生理的变化。结果对于健康犬在气道峰压相似的情况下,APRV模式时平均气道压和肺动脉压显著高于IRV时。对血流动力学、血气和呼吸力学的影响两者差别不显著。APRV模式下氧运输量和PaO2/FiO2值较IRV对显著高。对于ALI犬APRV时的气道峰压显著低于IRV时,平均气道压两者相似,PaO2/FiO2则是APRV时显著高于IRV时。结论APRV与IRV相比,前者氧合功能显著优于后者,且气道峰压较低,更适用于ALI者。
To compare the effects of VC-IRV and APRV on cardiorespiratory function in healthy and acute lung injury (ALI) dogs. Methods Healthy and ALI model (dogs) were ventilated with IRV and APRV respectively to observe the change of hemodynamics, blood gas and respiratory physiology. Results When the peak airway pressure was similar in healthy dogs, mean airway pressure and pulmonary arterial pressure in APRV model were significantly higher than those in IRV. There was no significant difference between hemodynamics, blood gas and respiration mechanics. Oxygen delivery and PaO2 / FiO2 values were significantly higher in the APRV model than in the IRV pair. When the peak airway pressure of APRV in ALI dogs was significantly lower than that of IRV, the average airway pressure was similar and the PaO2 / FiO2 was significantly higher than that of IRV when APO2 was in API. Conclusion Compared with IRV, APRV is superior to the latter in oxygenation function and lower peak airway pressure, which is more suitable for ALI.
