目的分析登革热并发心功能损害的表现特征。方法对2007年本院收治的126例登革热患者的临床资料进行回顾性分析。通过检测心肌酶天门冬氨酸转氨酶(AST)、乳酸脱氢酶(LDH)、肌酸磷酸肌酶(CK)、α-羟丁酸脱氢酶(HBD)和心电图检查,分析登革热时心功能受损害情况。结果 126例登革热出现心功能损害者共107例(84.9%),其中心肌酶谱异常者共98例(占77.8%,AST升高85例,LDH升高96例,CK升高64例,HBD升高86例)。98例中有合并心电图异常者48例与无心电图异常者50例,两组各酶升高幅度比较有明显差异。126例登革热共有心电图异常57例(45.2%),主要为心肌损害(T波低平或/和S-T段改变)和心律失常(窦缓、窦速、窦性心律不齐、室性早博)表现。结论心肌受损在登革热发病过程中是一个多发和较常见表现,是本病发展中多系统损害的局部表现之一;心肌酶升高幅度与心电图改变结合分析,可综合判断心脏损伤程度。
Objective To analyze the manifestations of dengue fever complicated by cardiac dysfunction. Methods The clinical data of 126 dengue patients admitted to our hospital in 2007 were analyzed retrospectively. The cardiac function of dengue fever was analyzed by detecting myocardial enzymes such as AST, LDH, CK, HBD and ECG. Damage situation. Results A total of 107 cases (84.9%) had dengue fever, of which 98 cases had abnormal myocardial enzymes (77.8%, 85 cases with elevated AST, 96 cases with elevated LDH, 64 cases with elevated CK, HBD Elevated 86 cases). 98 patients with ECG abnormalities in 48 cases and no ECG abnormalities in 50 cases, the two groups increased the rate of each enzyme was significantly different. Conclusions Myocardial damage is a frequent and common manifestation of dengue fever. It is one of the local manifestations of multi-system lesion in the development of dengue fever. Combined with the increase of myocardial enzyme and electrocardiogram, the degree of heart damage can be comprehensively evaluated.
