目的:探讨能够客观反映血浆异丙酚量效关系的脑电指标。方法:ASAⅠ~Ⅱ级无心脑疾患的择期手术病人11例,腰麻完善后静脉给予异丙酚镇静,监测脑电活动,给药后定时测定血浆异丙酚浓度,将测得的异丙酚浓度与有关脑电指标作相关分析。结果:脑电双频指数(BIS)、95%边缘频率(SEF)、中位脑电频率与血浆异丙酚浓度呈负相关,其相关系数r分别为-07092、-04160及-02930,而脑电总功率、绝对δ功率、绝对θ功率、绝对α功率与血浆异丙酚浓度呈正相关,其r分别为03072、03112、03237和03876(P值均≤00001)。结论:BIS、SEF、绝对α功率均与血浆异丙酚浓度相关性较好,可结合临床作为异丙酚的镇静程度监测指标,并可用于异丙酚的药效学研究,其中BIS比较稳定、直观、与异丙酚浓度的相关性,优于其他脑电指标。
Objective: To explore the EEG index that can objectively reflect the dose-effect relationship of plasma propofol. Methods: Seventy - one patients undergoing ASA Ⅰ ~ Ⅱ elective surgery without encephalopathy were treated with propofol sedation after intravenous anesthesia. EEG activity was monitored. Plasma propofol concentration was measured regularly after administration. Phenol concentration and related EEG analysis. Results: The bispectral index (BIS), 95% edge frequency (SEF), median encephalofrequency were negatively correlated with plasma propofol concentration, and the correlation coefficients r were -0.792, -0.4160 and -0.2930, while the total EEG, absolute δ power, absolute θ power, absolute α power and plasma propofol concentration were positively correlated, and their r were 0 3072,0 3112,0 3237 and 0 3876 (P value is less than or equal to 0.0001). Conclusions: BIS, SEF and absolute α power have good correlation with plasma propofol concentration, which can be combined with clinical monitoring indicators of sedation level of propofol, and can be used for the pharmacodynamic study of propofol, of which BIS is relatively stable , Intuitive, with the concentration of propofol, better than other EEG indicators.
