目的:探讨慢性阻塞性肺疾病(COPD)合并II型呼吸衰竭患者采用BiPAP呼吸机治疗的临床效果。方法:将我院2011年5月~2014年5月接诊的108例COPD合并II型呼吸衰竭患者作为研究对象,随机均分为研究组与对照组,各自54例。其中对照组患者采取药物、氧疗等常规处理,而研究组患者在对照组方式上加用BiPAP呼吸机治疗。观察记录两组患者治疗前、治疗后(72h)血气分析情况、心率与呼吸频率变化情况,血气分析主要包括PH值、氧分压(PaO2)、二氧化碳分压(PaCO2)情况,并进行对比分析。结果:两组患者治疗后(72h)PH值、氧分压、二氧化碳分压以及心率与呼吸频率相较于治疗前皆有明显变化,组内对比差异有统计学意义(P<0.05),但研究组治疗后变化更明显,组间对比差异也有统计学意义(P<0.05)。结论:慢性阻塞性肺疾病合并II型呼吸衰竭患者采用BiPAP呼吸机治疗可以取得比较良好的效果,能更好地改善患者的心率、呼吸频率、氧分压、二氧化碳分压等指标情况,值得借鉴。
Objective: To investigate the clinical effect of BiPAP ventilator in patients with chronic obstructive pulmonary disease (COPD) and type II respiratory failure. Methods: A total of 108 patients with COPD and type II respiratory failure admitted to our hospital from May 2011 to May 2014 were randomly divided into study group and control group, with 54 patients in each group. Patients in the control group were treated with drugs and oxygen therapy routinely, while patients in the study group were treated with BiPAP ventilator in the control group. The blood gas analysis, heart rate and respiratory rate before and after treatment (72h) were observed and recorded. The blood gas analysis mainly included PH value, PaO2 and PaCO2, and compared them . Results: Compared with before treatment, PH value, partial pressure of oxygen, partial pressure of carbon dioxide and heart rate and respiration rate in both groups after treatment were significantly different (P <0.05), but the differences were statistically significant The study group changed more obviously after treatment, the difference between the groups was also statistically significant (P <0.05). CONCLUSIONS: BiPAP ventilator therapy in patients with chronic obstructive pulmonary disease and type II respiratory failure can achieve better results, and can better improve the patient’s heart rate, respiratory rate, oxygen partial pressure, carbon dioxide partial pressure and other indicators, it is worth learning from .
