目的 :探讨剖宫产和顺产对早产儿血气、电解质、肾功能及心肌酶指标的影响。方法 :回顾性分析2013年8月-2015年10月在我院出生的早产儿的临床资料。根据其出生方式分为剖宫产组和顺产组,比较两组早产儿出生后Apgar评分、血气、电解质、肾功能及心肌酶指标的差异,观察两组早产儿并发症发生率。结果 :顺产组早产儿出生后1min和5min的Apgar评分分别为9.82±0.54分和9.95±0.62分,与剖宫产组早产儿比较无明显差别;两组早产儿Hb、PLT、SpO2、P_(ET)CO2和CVP水平均无明显差别;两组早产儿血钠、血氯、尿素氮、肌酐和CKMB水平均无明显差别;顺产组吸入性肺炎和颅内出血发生例数分别为4例和1例,剖宫产组吸入性肺炎和颅内出血发生例数分别为1例和0例,两组早产儿并发症的发生率无明显差别。结论 :剖宫产和顺产对早产儿血气、电解质、肾功能及心肌酶指标均无明显影响,临床可根据孕妇具体情况选取合适的分娩方式。
Objective: To investigate the effect of cesarean section and delivery on the blood gas, electrolytes, renal function and myocardial enzymes in preterm infants. Methods: The clinical data of premature infants born in our hospital from August 2013 to October 2015 were retrospectively analyzed. The cesarean section group and the cesarean section group were divided according to their birth patterns. The differences of Apgar score, blood gas, electrolyte, renal function and myocardial enzymes between the two groups were compared after birth to observe the incidence of complications in both groups. Results: The Apgar scores of preterm infants born at 1 minute and 5 minutes after birth were 9.82 ± 0.54 and 9.95 ± 0.62, respectively, which were not significantly different from those of cesarean section preterm infants. The levels of Hb, PLT, SpO2 and P_ ET) had no significant difference in the levels of CO 2 and CVP. There was no significant difference in serum sodium, blood chlorine, urea nitrogen, creatinine and CKMB between the two groups. The incidence of asymptomatic pneumonia and intracranial hemorrhage were 4 and 1 For example, the incidence of aspiration pneumonia and intracranial hemorrhage in the cesarean section group were 1 case and 0 case, respectively. There was no significant difference in the incidence of complications between the two groups. Conclusion: Cesarean section and delivery have no significant effect on the blood gas, electrolytes, renal function and myocardial enzymes in preterm infants. According to the specific conditions of pregnant women, we can choose the appropriate mode of delivery.
