目的:研究再生障碍性贫血(AA)患者治疗前后血液生化指标的变化,分析AA患者的治疗对生化指标的影响。方法:2007~2010年来我院门诊就诊的AA患者共337例,按照轻、中、重、极重度AA患者的标准收集病例资料。比较患者总蛋白(TP)、白蛋白(ALB)、球蛋白(GLB)、白/球比(A/G)、前白蛋白(P-ALB)、葡萄糖(Glu)、肌酐(CREA)、尿素(UREA)、钾(K)、总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)等指标在治疗前后的变化。结果:AA患者治疗前,极重度贫血患者的TP、ALB、P-ALB、GLB、Glu与其他程度的贫血者之间的差异均有统计学意义(P<0.05);重度贫血者的TP、ALB、P-ALB与轻、中度贫血者的差异具有统计学意义(P<0.05);中度贫血病人的ALB、P-ALB与轻度贫血者比较差异无统计学意义(P>0.05)。AA患者治疗后,除了CREA、UREA、K之外,其他的在轻、中、重和极重度AA患者之间比较的差异均具有统计学意义(P<0.05)。治疗后,在各种程度贫血的病人血清中TP变化显著,ALB、P-ALB在重度和极重度贫血病人变化最为明显,A/G、Glu在极重度贫血的变化显著。结论:经过治疗AA患者的部分血液生化指标有显著的升高,主要是TP、ALB、P-ALB、GLB以及Glu,对于重度和及极重度贫血患者尤其明显。
Objective: To study the changes of blood biochemical indexes in patients with aplastic anemia (AA) before and after treatment, and to analyze the influence of the treatment of AA patients on the biochemical indexes. Methods: A total of 337 patients with AA in our outpatient department from 2007 to 2010 were collected according to the standard of patients with mild, moderate, severe and severe AA. The levels of TP, ALB, GLB, A / G, P-ALB, Glu, creatinine (CREA) (UREA), potassium (K), total bilirubin (TBIL), direct bilirubin (DBIL), indirect bilirubin (IBIL) and other indicators before and after treatment changes. Results: Before treatment, the differences of TP, ALB, P-ALB, GLB, Glu and other levels of anemia in patients with very severe anemia were statistically significant (P <0.05) ALB, P-ALB and mild to moderate anemia were statistically significant (P <0.05). There was no significant difference in ALB, P-ALB and mild anemia among patients with moderate anemia (P> 0.05) . After treatment with AA, other than the CREA, UREA, K, other in mild, moderate, severe and very severe AA patients were statistically significant differences (P <0.05). After treatment, the serum TP in patients with various degrees of anemia changed significantly. The changes of ALB and P-ALB in patients with severe and severe anemia were the most obvious. The changes of A / G and Glu in patients with severe anemia were significant. CONCLUSIONS: Some of the blood biochemical parameters in patients with AA have been significantly elevated, mainly TP, ALB, P-ALB, GLB and Glu, especially for patients with severe and very severe anemia.
