目的:探讨术中实时超声辅助下显微手术切除脑深部海绵状血管瘤的临床效果。方法回顾性分析2011年1月至2016年6月新疆生产建设兵团第四师医院神经外科(5例)及北京大学第一医院神经外科(3例)收治的8例脑深部海绵状血管瘤患者的临床资料。8例患者均在全身麻醉下行显微外科手术切除,术中引入超声对病变实时定位,选择距离病变位置最近的功能哑区及合适的手术入路,分离海绵状血管瘤的边界并分块或完整切除。术后对患者功能状态进行评估及随访。结果8例患者手术均顺利完成,术中及术后影像学检查均显示病变全切。病理均证实为海绵状血管瘤。术后患者神经功能均未再进一步恶化,5例局灶神经功能症状患者均获得不同程度恢复,3例癫痫表现患者术后随访均未再复发。结论术中实时超声能够精准定位,在辅助显微手术切除脑深部海绵状血管瘤具有重要作用。“,”Objective To evaluate the clinical effect of intraoperative ultrasound assisting microsurgery in resecting the deep cerebral cavernous malformation.Methods Data of eight cases with deep cavernous malformation in Xinjiang Production and Construction Corps Fourth Division Hospital from Jan .2011 to Jun. 2016 were retrospectively analyzed.Microsurgeries were all successfully performed in these 8 cases by intro-ducing intraoperative ultrasound in helping localization and selecting the suitable surgical approach .The bor-der of lesions were all carefully dissected and totally removed .Neurological function was evaluated and patients were followed up.Results All 8 patients recovered successfully without deterioration of neurological function.Intraoperative and postoperative image showed the lesions were all totally removed .Cavernous malformation was confirmed by pathological examination .Five patients with preoperative neurological deficits all recovered well and 3 patients with epilepsy were all free of recurrence during the follow-up period. Conclusion Intraoperative ultrasound is useful in precise localization of lesion and assisting resecting the deep cerebral cavernous malformation.
