目的对四川省某三甲医院1986年-2013年4月期间22023例住院死亡患者病案首页信息进行分类统计,对不同时期死亡疾病顺位进行相关分析。方法分析22023例住院死亡病例病死率及死亡疾病排序,分类资料采用频数和构成比进行描述。不同性别病死率差异比较运用χ2检验进行分析。运用SPSS 13.0软件进行统计分析。结果随着年龄的增长,病死率总体呈现先略有降低,再逐渐升高的趋势。从1986年-2013年4月变化来看,病死率总体呈现下降趋势。病死率在各个年龄段呈现出1986年-1996年组>1997年-2004年组>2005年-2013年4月组,依次为3.38%,2.60%和2.02%。男性和女性的病死率分别为3.07%和1.73%,差异有统计学意义(χ2=1754.466,P=0.000)。不同时期,住院患者死亡疾病顺位有所不同。结论随着社会经济的发展,居民健康状况得到改善,住院病死率呈下降趋势。但死亡疾病变化显示,肿瘤及心脑血管病等慢性非传染性疾病已居于主导地位,且死亡高峰出现于60岁以上的老年人。提示卫生预防重点应在于改善环境,倡导健康生活方式,以及对老年人群应予以更多关注。
Objective To classify and record the information of 22023 inpatients with inpatients from 1986 to 2013 in a Sichuan province from January 1986 to April 2013, and to analyze the rank order of death diseases in different periods. Methods The mortality and mortality of 22,023 hospitalized cases were analyzed. The classification data were described by frequency and composition ratio. Differences in mortality by gender were compared using χ2 test. Using SPSS 13.0 software for statistical analysis. Results With age, the overall mortality showed a slight decrease first, and then gradually increased. From 1986 to 2013 changes in April, the overall mortality showed a downward trend. The case-fatality rate of all age groups showed 1986-1996 group> 1997-2004 group> 2005-April 2013 group, followed by 3.38%, 2.60% and 2.02% respectively. Mortality rates of males and females were 3.07% and 1.73%, respectively, with significant differences (χ2 = 1754.466, P = 0.000). In different periods, inpatient deaths vary slightly. Conclusion With the development of society and economy, the residents’ health status has been improved and the in-hospital mortality rate has been declining. However, the change of death disease shows that chronic non-communicable diseases such as cancer and cardiovascular and cerebrovascular diseases are in the leading position, and the peak of death appears in the elderly over 60 years old. This suggests that health prevention should focus on improving the environment, advocating healthy lifestyles, and paying more attention to the elderly.
