目的 探讨脐静脉血治疗难治性消化系统疾病的作用 ,选择肝硬化食管胃底静脉曲张破裂出血、肝硬化并发腹水、重症溃疡性结肠炎、难治性腹泻并重度营养不良病例进行治疗。方法 从正常妊娠足月分娩后收集无菌脐静脉血 ,经血型鉴定和交叉配血相合的脐静脉血给消化系病患者静脉输注 ,观察临床疗效。结果 肝硬化并上消化道出血 6例 ,静脉输注脐血 4~ 6次 ,每次输血量 110~ 180ml,平均输脐血量 6 80ml,有效止血并临床好转出院 ,2年内未再复发者 5例 ,无效 1例。肝硬化复发性大量腹水 5例 ,静脉输注脐血 6~ 8次 ,平均输脐血 10 80ml,5例腹水均消失 ,身体一般状况明显恢复 ,血浆白蛋白正常。重症溃疡性结肠炎 5例 ,输脐血 5~ 6次 ,平均输脐血量 880ml,5例均临床治愈。难治性腹泻 3例经回输脐血 4~ 6次 ,平均输注 5 6 8ml,临床治愈。结论 脐血中富含对人体有用的多种成分 ,对消化系统难治性疾病的综合支持治疗很有价值 ,有条件情况下采用此疗法能取得特殊的效果。
Objective To investigate the effect of umbilical cord blood in the treatment of refractory digestive diseases and to select the patients with cirrhosis and esophageal varices bleeding, liver cirrhosis complicated with ascites, severe ulcerative colitis, refractory diarrhea and severe malnutrition. Methods Umbilical cord blood was collected from patients with normal umbilical cord blood after delivery. Umbilical venous blood was collected from patients with digestive system diseases through blood type identification and cross matching with blood. The clinical effects were observed. Results In cirrhosis and upper gastrointestinal bleeding in 6 cases, intravenous infusion of cord blood 4 to 6 times, each transfusion volume 110 ~ 180ml, the average amount of cord blood delivery 6 80ml, effective hemostasis and clinical improvement out of hospital, no recurrence within 2 years 5 cases, 1 case invalid. 5 cases of cirrhosis recurrent massive ascites, intravenous infusion of cord blood 6 to 8 times, an average of 10 80ml cord blood, 5 cases of ascites disappeared, the general condition of the body recovered significantly, normal plasma albumin. Severe ulcerative colitis in 5 cases, umbilical cord blood 5 to 6 times, the average amount of cord blood 880ml, 5 cases were clinically cured. Refractory diarrhea in 3 cases of umbilical cord returned 4 to 6 times, the average infusion of 568ml, clinical cure. Conclusion Umbilical cord blood is rich in various components useful to the human body and is valuable for the comprehensive support and treatment of refractory diseases of the digestive system. Under certain conditions, this therapy can achieve special effects.
