目的 :探讨经牙槽嵴顶入路液压提升法提升上颌窦底黏膜并植骨同期种植的临床效果。方法 :选取经牙槽嵴顶入路液压提升法提升上颌窦底黏膜并植骨同期进行种植体植入术的患者25例,其中男15例,女10例,年龄40~62岁,均为上颌后牙单颗缺失,缺牙区剩余上颌窦骨高度为(4.25±1.12)mm,进行临床检查和影像学检查,评估其临床效果。结果:25例病例上颌窦底提升高度为(8.50±2.12)mm,术中未发现上颌窦底黏膜破损,同期植入Bio-Oss骨粉并植入种植体25颗。1例患者因种植体感染于术后3周脱落,后检查发现该患者患有糖尿病,暂时放弃再次种植。其余24例患者术后观察未发现明显并发症和不良反应。随访12~18个月,所有种植体均行使良好功能。结论:液压提升法提升上颌窦底黏膜并植骨同期种植安全、可靠,手术创伤小,较传统冲顶式内提升法更加灵活,并可避免敲击震荡给患者带来的恐惧和不适,临床效果较好。
Objective: To investigate the clinical effect of raising the maxillary sinus mucosa with bone graft by hydraulically raising the alveolar crest. Methods: Twenty-five patients, including 15 males and 10 females, aged 40-62 years, were enrolled in this study. The patients were enrolled in this study. The maxillary posterior teeth were singly deleted, and the height of remaining maxillary sinus in edentulous area was (4.25 ± 1.12) mm. The clinical and radiographic examinations were performed to evaluate the clinical effect. Results: The lifting height of maxillary sinus floor was (8.50 ± 2.12) mm in 25 cases. No damage to maxillary sinus mucosa was found during operation. Bio-Oss bone powder was implanted in the same period and 25 implants were implanted. One patient died of implant infection 3 weeks after surgery, after examination found that the patient suffering from diabetes, to temporarily give up planting again. The remaining 24 patients observed no significant complications and adverse reactions. All patients underwent 12 to 18 months of follow-up with good function. CONCLUSION: The hydraulic lifting method can enhance the safety of the maxillary sinus mucosa and bone graft in the same period. The trauma is less invasive than the traditional top lift method and can avoid the fear and discomfort caused by percussion shock. The clinical effect better.
