目的探讨肾脏CT灌注参数在60岁以上健康老年人中的变化。方法30例正常人按年龄分两组:I组40~59岁,15例,II组60~79岁,15例,全部采用Siemens16层螺旋CT行双侧肾脏灌注扫描。采集数据经后处理,获得肾脏血流灌注图像和时间?密度曲线(TDC),通过感兴趣区获得皮髓质的不同灌注参数,包括肾血流量(BF)、肾血容量(BV)、肾脏对比剂平均通过时间(MTT)和肾脏强化高峰时间(TTP)。结果60岁以上组肾皮质的BF值较40~59岁组明显减低,差异有统计学意义(P<0.05);MTT值在60岁以上组有增加趋势,但与40~59岁差异均无统计学意义(P>0.05)。肾髓质灌注参数两组间差异均无统计学意义(P>0.05),但BV和MTT值在60岁以上组有增加趋势。结论年龄老化对肾脏血流的影响主要是皮质BF值显著下降;为临床了解肾脏病理生理变化提供量化依据。
Objective To investigate the changes of renal CT perfusion parameters in healthy elderly over 60 years old. Methods Thirty healthy controls were divided into two groups according to age: group I 40 to 59 years old, group I, group 60 to 79, group I, and group 15 all using Siemens 16 - slice spiral CT with bilateral renal perfusion scan. The collected data were post-processed to obtain the renal perfusion images and time density curve (TDC). Different perfusion parameters of cortex were obtained through the region of interest, including renal blood flow (BF), renal blood volume (BV) Mean contrast time (MTT) and renal peak tightening time (TTP). Results The BF value of renal cortex in the group over 60 years old was significantly lower than that of the group 40 to 59 years old (P <0.05), while the MTT value increased in the group over 60 years old, but no significant difference between 40 and 59 years old Statistical significance (P> 0.05). There was no significant difference in the parameters of renal medullary perfusion between the two groups (P> 0.05). However, the values of BV and MTT increased in the group over 60 years old. Conclusion The effect of aging on renal blood flow is mainly due to the significant decrease of cortical BF value. It provides a quantitative basis for clinical understanding of renal pathophysiological changes.
