目的血清分泌型卷曲相关蛋白5(SFRP5)是新发现的抗炎型脂肪因子,本研究旨在探索SFRP5与儿童代谢综合征(MS)的相关性,并研究经过综合减重干预后SFRP5的变化。方法将111名肥胖儿童随机分为肥胖伴MS组(n=50)、不伴MS组(n=61),正常体重儿童49名作为对照组,对每位儿童进行身高、体重、腰围、血压等测量,并行血脂、血糖、肝肾功等化验,留取静脉血清标本测血清胰岛素、SFRP5、Wnt5a、hsCRP(超敏C反应蛋白)、瘦素、脂联素水平等。对31名MS儿童进行综合减重方案干预6个月,并分析其减重前后各指标变化。结果 SFRP5在肥胖伴MS患儿中水平较另两组水平低(P<0.05)。矫正年龄及BMI-SDS后,SFRP5与瘦素/脂联素(LAR)、胰岛素抵抗指数(Ln HOMA-IR)仍为负相关(r’=-0.250,r’=-0.284,P均<0.05),与脂联素呈正相关(r’=0.339,P<0.05)。31名经综合减重干预后患儿的BMI、血压、HOMA-IR、甘油三酯、瘦素、LAR较前均降低,脂联素、SFRP5较前升高(P<0.05)。结论血清SFRP5水平与中心性肥胖、脂质代谢紊乱、高血压、高胰岛素血症密切相关,在儿童MS的发病中有保护作用。
Objective To investigate the relationship between SFRP5 and childhood metabolic syndrome (MS) and to investigate the changes of SFRP5 after the intervention of total weight loss (SFRP5), a newly discovered anti-inflammatory adipokine. . Methods 111 obese children were randomly divided into obesity group with MS (n = 50), without MS group (n = 61) and 49 normal weight children as control group. The height, weight, waist circumference, blood pressure Serum insulin, SFRP5, Wnt5a, hsCRP (hsCRP), leptin and adiponectin were measured in serum, parallel blood lipid, blood glucose, liver and kidney function. A total of 31 MS children underwent weight loss regimen for 6 months and analyzed their changes before and after weight loss. Results The level of SFRP5 in obese MS patients was lower than that in the other two groups (P <0.05). SFRP5 was still negatively correlated with Ln and Ln HOMA-IR after correction of age and BMI-SDS (r ’= - 0.250, r’ = - 0.284, all P <0.05 ), And adiponectin was positively correlated (r ’= 0.339, P <0.05). Eleven children with BMI, blood pressure, HOMA-IR, triglyceride, leptin and LAR after intervention were lower than before and adiponectin and SFRP5 were higher than those before (P <0.05). Conclusion Serum SFRP5 levels are closely related to central obesity, lipid metabolism disorder, hypertension and hyperinsulinemia and may play a protective role in the pathogenesis of childhood MS.
