目的 探讨妊娠期高血压疾病患者血清血管细胞黏附分子 1(VCAM 1)和白细胞介素 6 (IL 6 )水平的变化 ,二者的关系和VCAM 1在子宫胎盘床的表达 ,及其与妊娠期高血压疾病发病机制的关系。方法 (1)于2 0 0 0年 3月至 2 0 0 3年 2月 ,青岛大学附属医院采用双抗体夹心酶联免疫吸附法 (ELISA)测定了 6 0例妊娠期高血压疾病患者 (其中妊娠期高血压 2 0例、子疒间 前期轻度及重度各 2 0例 ) ,35例正常晚期妊娠妇女血清VCAM 1和IL 6水平。 (2 )采用免疫组织化学染色法检测了 6 0例妊娠期高血压疾病患者、30例正常妊娠妇女子宫胎盘床螺旋动脉VCAM 1的表达。结果 (1)妊娠期高血压疾病组血清VCAM 1水平明显高于正常妊娠组 (P <0 0 5 )。 (2 )子宫胎盘床螺旋动脉滋养细胞VCAM 1表达 ,妊娠期高血压疾病组阳性表达率较正常妊娠组明显降低 (P <0 0 5 )。 (3)妊娠期高血压疾病组血清IL 6含量明显高于正常妊娠组 (P <0 0 5 ) ,VCAM 1与IL 6水平呈明显正相关 (r =0 6 3,P <0 0 1)。结论 妊娠期高血压疾病患者血清中VCAM 1和IL 6水平明显升高 ,而子宫胎盘床VCAM 1表达减少。VCAM 1和IL 6参与了妊娠期高血压疾病的发病机制。二者在子疒间 前期患者血清中水平呈正相关。
Objective To investigate the changes of serum vascular cell adhesion molecule 1 (VCAM 1) and interleukin 6 (IL 6) levels in patients with gestational hypertension and the relationship between them and the expression of VCAM 1 in uterine placenta bed and its relationship with gestational age Hypertensive disease pathogenesis. Methods (1) From March 2000 to February 2003, Qingdao University Affiliated Hospital of Qingdao University measured 60 cases of hypertensive disorder complicating pregnancy with double antibody sandwich enzyme-linked immunosorbent assay (ELISA 20 cases of pregnancy-induced hypertension and 20 cases of pre-and post-nocturnal hyperplasia) and serum VCAM-1 and IL-6 levels in 35 normal pregnant women. (2) Immunohistochemical staining was used to detect the expression of VCAM-1 in the uterine placenta helix in 60 pregnant women with gestational hypertension and 30 normal pregnant women. Results (1) The level of serum VCAM-1 in gestational hypertension group was significantly higher than that in normal pregnancy group (P <0.05). (2) The expression of VCAM-1 in uterine spiral placenta of spiral artery trophoblast cells was significantly lower in gestational hypertension group than that in normal pregnancy group (P <0.05). (3) Serum levels of IL-6 in gestational hypertension group were significantly higher than those in normal pregnancy group (P <0 05), VCAM 1 and IL 6 levels were positively correlated (r = 0 6 3, P 0 01) . Conclusion Serum levels of VCAM 1 and IL 6 in patients with gestational hypertension are significantly increased, while the expression of VCAM 1 in uterine placenta is decreased. VCAM 1 and IL 6 are involved in the pathogenesis of hypertensive disorders of pregnancy. Both of them had a positive correlation with the level of serum in the patients with preeclampsia.
