目的了解白银市流行性感冒(流感)监测网络运行情况,评估流感监测质量,探索市级评估方法。方法依据《全国流感监测工作质量评估方案》,结合市级实际情况,选取国家级哨点医院、市级网络实验室和暴发监测管理三方面指标,对白银市各年度流感监测质量进行量化评分。结果 2009 2015年,白银市流感监测质量综合评估平均得分为73.25分,2015年最高,为88.75分。两家哨点医院流感样病例(ILI)及时报告周次比例一直在90.00%以上,无缺报、漏报,采样合格周次比例2013年以来达90.00%以上(χ_(趋势)~2=42.613,P=0.000;χ_(趋势)~2=17.356,P=0.000),标本送检及时率分别为53.91%和40.89%。市级网络实验室核酸检测及时率呈逐年上升趋势(χ_(趋势)~2=272.189,P=0.000),送检毒株复核一致率从73.08%上升至98.00%以上(χ_(趋势)~2=20.129,P=0.000),阳性标本亚型鉴定比例、接受国家盲样考核正确率达100.00%,但流感病毒的鸡胚分离能力需要提高,向国家流感中心及时送检毒株数量不足。全市7年报告28起ILI/流感暴发疫情,监测管理情况良好。结论白银市流感监测质量明显提高,但仍需加强哨点医院ILI监测,市级网络实验室在原有工作基础上进一步提高流感病毒的鸡胚分离率。
Objective To understand the operation of influenza (influenza) surveillance network in Baiyin, assess the quality of influenza surveillance and explore municipal assessment methods. Methods According to the “National Influenza Surveillance Quality Evaluation Program” and the actual municipal level, we selected three indicators of national sentinel hospitals, municipal network laboratories and outbreak monitoring and management to quantify the annual influenza surveillance quality in Baiyin. Results In 2009, the overall average score of flu surveillance quality in Silver City was 73.25 in 2015, the highest in 2015, at 88.75. The proportion of flu-like cases (ILI) reported weekly in the two sentinel hospitals has been above 90.00%, with no missing or omissions reported, and the weekly sampling pass rate has reached more than 90.00% (χ_ (trend) ~ 2 = 42.613 since 2013) P = 0.000; χ_ (trend) ~ 2 = 17.356, P = 0.000). The time of submission and examination were 53.91% and 40.89% respectively. The detection rate of nucleic acid in municipal network laboratories showed an upward trend year by year (χ_ (trend) ~ 2 = 272.189, P = 0.000). The coincidence rate of drug review increased from 73.08% to over 98.00% (χ_ (trend) = 20.129, P = 0.000). The proportion of positive subtype was positive, and the correct rate of receiving blind samples was 100.00%. However, the ability of influenza virus to separate chicken embryo needs to be improved, and the number of strains sent to the National Influenza Center in time was not enough. The city’s 7-year report 28 ILI / flu outbreaks, monitoring and management in good condition. Conclusion The quality of influenza surveillance in Baiyin City increased significantly. However, monitoring of ILI in sentinel hospitals should be strengthened. Municipal network laboratories should further increase the isolation rate of chicken embryos based on the previous work.
