目的了解2013—2015年我国手足口病死亡病例的社会经济负担。方法应用人力资本法估计不同年龄组手足口病死亡病例早亡所导致的人均生产力损失(损失总收入的现值)。使用前期手足口病死亡病例治疗期间人均经济负担以及全国法定传染病报告的手足口病死亡病例信息,计算2013—2015年全国手足口病死亡病例社会经济负担,并采用不同贴现率对结果进行敏感性分析。结果 2013—2015年全国手足口病死亡病例数分别为252、501和129例,以男性为主,年龄以1~3岁为主。不同年龄组手足口病死亡病例的早亡所致生产力损失在55.1万~95.5万元之间,人均社会经济负担在56.5万~97.0万元之间。2013—2015年早亡总损失分别为14 491.9、31 159.1和8 316.7万元,社会经济负担分别为14 858.6、31 896.7和8 510.4万元。敏感性分析中,当贴现率设置为0时,2013—2015年早亡损失增长2.0倍,社会经济负担增长了2.0倍;当贴现率设置为5%时,2013 2015年早亡损失下降了47.2%,社会经济负担下降了46.1%。结论手足口病所致死亡带来较重的社会经济损失,应采用疫苗等措施预防手足口病。
Objective To understand the socioeconomic burden of HFMD deaths in China from 2013 to 2015. Methods The human capital method was used to estimate the loss of per capita productivity (the present value of total loss of income) caused by premature death of HFMD in different age groups. Using the information on per capita economic burdens of HFMD deaths during the pre-treatment period and the reported deaths from hand-foot-mouth disease from national notifiable infectious diseases, the socioeconomic burden of deaths from HFMD in 2013-2015 was calculated and the results were sensitized using different discount rates Sexual analysis. Results The number of HFMD deaths in China during the period from 2013 to 2015 were 252,501 and 129, respectively, mainly male, with the majority being aged 1 to 3 years old. The loss of productivity due to premature death of hand, foot and mouth disease in different age groups was between 551,000 and 955,000 yuan, and the per capita social and economic burden was between 565,000 and 970,000 yuan. The total loss from early death in 2013-2015 was 14,491.9, 31,159.1 and 83.617 million respectively, with the social and economic burden of 14,858.6, 31,896.7 and 81,014,000 respectively. In the sensitivity analysis, when the discount rate is set as 0, the loss of premature deaths will increase by 2.0 times and the socio-economic burden by 2.0 times in 2013-2015. When the discount rate is set at 5%, the premature death loss in 2013-2015 will drop by 47.2% %, The social and economic burden has dropped by 46.1%. Conclusion Hand-foot-mouth disease causes heavy social and economic losses. Vaccination and other measures should be taken to prevent hand-foot-mouth disease.
