目的:探讨纳米碳淋巴结示踪技术在甲状腺乳头状癌手术Ⅵ区淋巴结清扫术中对甲状旁腺的保护作用。方法:回顾性分析2015-10-2016-05期间我院收治的148例甲状腺乳头状癌手术患者,其中术中应用纳米碳示踪技术的75例患者作为观察组,常规手术患者73例作为对照组,所有患者均行中央区(Ⅵ区)淋巴结清扫术。比较两组淋巴结清扫数量、转移淋巴结数、术后低钙血症发生率以及甲状旁腺损伤率。结果:两组患者年龄、性别、肿瘤大小、手术方式等一般情况比较差异无统计学意义(P>0.05)。甲状旁腺损伤率观察组明显低于对照组(观察组:5.33%,对照组:16.44%,P<0.05)。Ⅵ区淋巴结清扫数量比较2组差异有统计学意义[观察组:(6.51±4.65)枚/例,对照组:(4.42±3.10]枚/例,P<0.05]。结论:甲状腺乳头状癌术中使用纳米碳混悬液具有良好的淋巴结示踪效果,其甲状旁腺负显影的特点可以有效减少甲状旁腺误切。
Objective: To investigate the protective effect of nanocarbon lymph node tracing on parathyroid glands in Ⅵ lymph node dissection of papillary thyroid carcinoma. Methods: A retrospective analysis of 148 cases of thyroid papillary carcinoma treated in our hospital during 2015-10-2016-05 was performed. Among them, 75 cases with intraoperative carbon nano-tracing were treated as observation group and 73 cases with conventional surgery as control Group, all patients underwent central zone (Ⅵ area) lymph node dissection. The number of lymph node dissection, the number of metastatic lymph nodes, the incidence of postoperative hypocalcemia and the parathyroid injury rate were compared between the two groups. Results: There were no significant differences in the general conditions such as age, sex, tumor size and operation between the two groups (P> 0.05). Parathyroid injury rate was significantly lower in the observation group than in the control group (observation group: 5.33%, control group: 16.44%, P <0.05). The number of lymph node dissection in Ⅵ region was significantly different between the two groups (6.51 ± 4.65 in control group and 4.42 ± 3.10 in control group, P <0.05) .Conclusion: Papillary thyroid carcinoma In the use of nano-carbon suspension has good lymph node tracing effect, the negative imaging of the parathyroid glands can effectively reduce the false negative parathyroid.
