目的 :研究脐血清中白细胞介素 3(IL 3)的表达水平并观察脐血输注预防肿瘤化疗诱发的白细胞 (WBC)和中性粒细胞 (ANC)减少的临床效果。方法 :应用双抗体夹心酶联免疫吸附试验 (ELISA)对 38份脐血清和 2 3份健康成人外周血清进行了IL 3水平测定 ;肿瘤患者 41例化疗末次用药后 2 4~ 72小时接受脐血输注 ,2 8例化疗中口服利血生(2 0mg ,每日 3次 )作为对照组 ,观察WBC及ANC的变化。结果 :脐血清中IL 3表达水平 [(6 38.3± 2 0 5 .7) μg/L]显著高于成人外周血 [(6 6 .8± 39.3) μg/L] (P <0 .0 0 1) ;脐血输注组患者化疗后WBC、ANC低于正常值的持续时间 ,与对照组相比显著缩短 ,ANC最低值显著高于对照组 ,感染发生率显著降低 ,生活质量改善率显著高于对照组。结论 :脐血清是IL 3的一个良好的天然来源 ;脐血输注预防化疗诱发的粒细胞减少症效果确切 ,值得进一步研究和推广应用 ,并可为治疗其他化学毒物及电离辐射诱发的骨髓抑制提供参考
Objective: To study the expression of interleukin-3 (IL-3) in cord serum and to observe the clinical effect of cord blood transfusion in preventing the decrease of white blood cell (WBC) and neutrophil (ANC) induced by chemotherapy. Methods: IL-3 levels were measured in 38 serum samples of umbilical cord blood and 23 healthy adult peripheral blood by double-antibody sandwich enzyme-linked immunosorbent assay (ELISA). Tumor patients received umbilical cord blood 24 to 72 hours after the last dose of chemotherapy Infusion, 28 cases of oral chemotherapy in reserpine (20mg, 3 times a day) as a control group, observed WBC and ANC changes. Results: The level of IL-3 in umbilical serum [(6 38.3 ± 2.57) μg / L] was significantly higher than that in adult peripheral blood [(66.8 ± 39.3) μg / L] 1). The durations of WBC and ANC lower than normal after chemotherapy in cord blood transfusion group were significantly shorter than those in control group, the lowest in ANC group was significantly higher than that in control group, the incidence of infection was significantly decreased, and the quality of life improvement rate was significantly Higher than the control group. Conclusion: Umbilical cord serum is a good natural source of IL-3. Umbilical cord blood transfusion can prevent chemotherapeutic-induced neutropenia and is worth further study and application. It can be used for the treatment of other chemical poisons and ionizing radiation-induced myelosuppression for reference
