目的 :说明PCNA含量及DNA含量联合检测在子宫内膜癌及癌前病变中的诊断意义 ;两种取材方法对结果的影响 ,以探讨临床工作采用新鲜标本作为早期诊断的辅助手段是可行的。方法 :采用流式细胞术联合检测 57例子宫内膜癌及各种增生的子宫内膜组织的PCNA及DNA含量。结果 :PCNA含量及DNA含量在正常子宫内膜、良性增生过长和不典型增生及内膜癌的差异有显著意义 (P <0 0 1) ,PCNA含量与DNA含量呈完全正相关 ,石蜡包埋标本及新鲜标本的两种取材方法PCNA含量、DNA含量比较无差异 (P >0 0 5)。结论 :PCNA含量及DNA含量、异倍体率随子宫内膜增生程度的加重呈递增趋势 ,其测定为内膜癌及癌前病变的早期诊断及鉴别诊断提供了一客观定量的指标。但两种检测值在各种病变之间均有部分病例有重叠交叉现象 ,联合检测可减少漏诊 ,并且采用新鲜标本作为早期诊断的辅助手段是可行的。
OBJECTIVE: To elucidate the diagnostic value of the combined detection of PCNA content and DNA content in endometrial carcinoma and precancerous lesions. The effects of two methods on the results are discussed to explore the feasibility of using fresh specimens as an aid in early diagnosis in clinical practice. Methods: The PCNA and DNA contents of 57 cases of endometrial carcinoma and various proliferative endometrial tissues were detected by flow cytometry. Results: There was a significant difference in PCNA content and DNA content between normal endometrium, benign hyperplasia, atypical hyperplasia and endometrial carcinoma (P <0.01). There was a positive correlation between PCNA content and DNA content. There were no differences in PCNA content and DNA content between the two methods of burial and fresh specimens (P> 0.05). Conclusions: The content of PCNA, DNA content and aneuploidy rate increase with the degree of endometrial hyperplasia, which provides an objective and quantitative index for the early diagnosis and differential diagnosis of endometrial carcinoma and precancerous lesions. However, there are overlap of crossover between the two detection values in various diseases. Combined detection can reduce the missed diagnosis, and it is feasible to use fresh specimens as an aid in early diagnosis.
