目的初步探讨改良吸宫术联合甲氨蝶呤(methtrexate,MTX)治疗表浅型子宫瘢痕妊娠(cesarean scar pregnancy,CSP)的有效性和安全性。方法回顾分析16例诊断为表浅型CSP患者,征得患者同意,在充分的开腹手术准备下,采用改良吸宫术联合MTX治疗,观察手术成功率、术中出血量、操作时间、住院时间以及随访情况。结果 16例患者均取得成功,手术操作时间6～8 min,术中出血65～120 mL,住院时间6～10 d,随访时间4～6个月,随访期间1例患者因阴道流血再次住院,经止血预防感染等保守治疗成功。结论改良吸宫术联合MTX治疗表浅型CSP是一种有效、安全、微创的方法。
Objective To investigate the efficacy and safety of modified inhalation combined with methotrexate (MTX) in the treatment of cesarean scar pregnancy (CSP). Methods A retrospective analysis of 16 cases of patients diagnosed as superficial CSP patients with consent of patients underwent adequate open surgery combined with MTX treatment, observation of the success rate of surgery, blood loss, operation time, hospitalization Time and follow-up situation. Results All the 16 patients were successful. The operation time ranged from 6 to 8 min. The intraoperative bleeding ranged from 65 to 120 mL. The duration of hospitalization was from 6 to 10 days. The follow - up duration ranged from 4 to 6 months. One patient was hospitalized again due to vaginal bleeding during follow - After hemostasis to prevent infection and other conservative treatment success. Conclusion Modified suction aspiration combined with MTX treatment of superficial CSP is an effective, safe and minimally invasive method.
