【目的】为探讨不同时期肝癌患者体内免疫杀伤肿瘤细胞的效应。【方法】将符合入选标准的肝癌患者抽取50例,按其病情的进展程度分为Ⅰ、Ⅱ、Ⅲ、Ⅳ期,同时测定不同时期CD8+T细胞及其分泌的细胞因子IFN-γ和穿孔素,并进行Spearman分析。【结果】①I、Ⅱ、Ⅲ、Ⅳ期肝癌中CD8+T细胞占淋巴细胞的百分比为(51.23±10.84)%、(46.23±8.52)%、(31.13±7.27)%、(22.68±5.76)%;CD8+T细胞表达IFN-γ占CD8+T细胞百分比分别为(65.84±11.47)%、(52.72±10.89)%、(46.57±8.64)%、(36.53±6.72)%;表达穿孔素占CD8+T细胞百分比分别为(65.32±11.69)%、(53.67±10.33)%,(41.66±8.67)%, (34.92±5.65)%;各时期肝癌两两相比,CD8+T细胞及其表达IFN-γ百分比、表达穿孔素的百分比均有统计学意义(P均<0.01).②经Spearman相关分析发现肝癌的分期与CD8+T细胞及其表达细胞因子IFN-γ、表达穿孔素的百分比呈负相关,秩相关系数分别为rs=-0.6137;rs=0.5718;rs=0.4654。【结论】随着疾病的进展,肝癌患者CD8+T细胞及其分泌的细胞因子IFN-γ和穿孔素均下降,具有杀伤肿瘤效应的免疫功能减弱。
【Objective】 To investigate the effect of immune-killing of tumor cells in patients with liver cancer at different stages. 【Methods】 Fifty patients with hepatocellular carcinoma who met the inclusion criteria were divided into Ⅰ, Ⅱ, Ⅲ and Ⅳ stages according to the degree of progression of the disease. The levels of CD8 + T cells and their secreted cytokines IFN-γ and perforation Prime, and Spearman analysis. 【Results】 ① The percentages of CD8 + T cells in lymphocytes in I, II, III and IV hepatocellular carcinoma were (51.23 ± 10.84)%, (46.23 ± 8.52)%, (31.13 ± 7.27)%, (22.68 ± 5.76)%, ; The percentages of CD8 + T cells expressing CD8 + T cells in CD8 + T cells were (65.84 ± 11.47)%, (52.72 ± 10.89)%, (46.57 ± 8.64)%, (36.53 ± 6.72)%, + T cells were (65.32 ± 11.69)%, (53.67 ± 10.33)%, (41.66 ± 8.67)% and (34.92 ± 5.65)%, respectively -γ, and the percentage of perforin were all statistically significant (all P <0.01) .②The Spearman correlation analysis showed that the staging of hepatocellular carcinoma was positively correlated with the percentage of CD8 + T cells and their expression of IFN-γ and perforin Negative correlation, rank correlation coefficient were rs = -0.6137; rs = 0.5718; rs = 0.4654. 【Conclusion】 With the progress of the disease, the CD8 + T cells and the secreted cytokines IFN-γ and perforin in HCC patients decreased, and the immune function with anti-tumor effect decreased.
