目的探讨锥形束CT(CBCT)在牙体牙髓病诊治中的应用价值。方法选择行CBCT检查的牙体牙髓病患者60例,对比数字根尖片,从判断牙根根折、根管数目、观察牙内外吸收情况、评价根管治疗术的治疗效果4个方面进行总结分析CBCT优势。结果 CBCT辅助判断根折:11例疑似根折患者中,CBCT检出9例,2例患者无根折;判断根管数目8例,准确率为100%;观察牙根内外吸收情况16例,检出11例;治疗中辅助定位根管13例,均显示良好;确定根管治疗失败原因7例:(1)根管填充不严密3例;(2)漏充根管2例;(3)根管钙化2例。所有行CBCT检查结果均经临床证实。结论 CBCT能够清晰显示根管的数目及走形,对牙内外吸收的情况反映灵敏,能够评价根管治疗的效果,帮助总结治疗失败的原因。 Objective To investigate the value of cone beam computed tomography (CBCT) in diagnosis and treatment of dental pulp disease. Methods Sixty patients with dental pulp diseases who underwent CBCT examination were selected and compared with digital root tips to summarize the results from four aspects: determining the number of root fractures and root canal, observing the internal and external absorption of teeth, and evaluating the effect of root canal therapy Analysis of CBCT advantages. Results CBCT assisted determination of root fracture: 11 cases of suspected root fracture patients, CBCT detected in 9 cases, 2 patients without root fracture; judge the number of root canal in 8 cases, the accuracy rate was 100%; Observation of tooth root internal and external absorption in 16 cases, (11 cases). In the treatment, 13 cases were assisted to locate the root canal, all of which showed good results. 7 cases of root canal failure were identified: (1) Root canal calcification in 2 cases. All CBCT examination results were clinically confirmed. Conclusion CBCT can clearly show the number of root canal and its shape, which is sensitive to the situation of internal and external absorption. It can evaluate the effect of root canal therapy and help to summarize the causes of treatment failure.
