目的探讨脉冲组织多普勒成像(PW-TDI)和潘生丁负荷试验对冠心病的早期诊断价值。方法选择冠心病患者73例(均为左前降支狭窄≥50%),设正常对照组25例。所有病例均经冠状动脉造影证实。应用PW-TDI分别测量两组静息状态及潘生丁负荷状态下左室前壁、前间隔的室壁基底段和中段Vs、VE、VA值。结果静息状态下,冠心病组Vs、VE较对照组减低,且有显著性差异(P<0.05)。单纯应用脉冲多普勒组织成像诊断冠心病敏感性80.3%、特异性72.1%。潘生丁负荷状态下,两组对应数据除VA外均差异明显增大,诊断冠心病敏感性93.6%、特异性86.1%。结论PW-DTI结合潘生丁负荷超声心动图对冠心病的早期诊断有肯定的应用价值。
Objective To investigate the value of pulsed Tissue Doppler Imaging (PW-TDI) and dipyridamole load test in the early diagnosis of coronary heart disease. Methods 73 patients with coronary heart disease (both left anterior descending artery stenosis ≥ 50%), 25 cases of normal control group. All cases were confirmed by coronary angiography. The basal and middle Vs, VE and VA values of left ventricular anterior wall and anterior septum were measured by PW-TDI at rest and dipyridamole respectively. Results In resting state, Vs and VE in coronary heart disease group were lower than those in control group, and there was significant difference (P <0.05). Pulse Doppler tissue imaging diagnosis of coronary heart disease with a sensitivity of 80.3%, a specificity of 72.1%. Under the condition of dipyridamole, the differences between the two groups of data except VA were significantly increased. The sensitivity and accuracy of diagnosing coronary heart disease were 93.6% and 86.1% respectively. Conclusions PW-DTI combined with dipyridamole stress echocardiography has certain value in the early diagnosis of coronary heart disease.
