目的 了解先天性心脏病患儿血浆降钙素基因相关肽(CGRP)及血浆内皮素(ET)的变化,探讨其在先天性心脏病继发的病理生理改变中的作用及其临床意义。方法 左向右分流型先天性心脏病患儿29 例,其中11例成功地行修补术,正常对照组25 例。用放射免疫均相竟争法分别检测血浆CGRP及ET水平。结果 先天性心脏病患儿术前血浆CGRP较对照组低(P< 0.01),而ET较对照组高(P< 0.01);患儿术后血浆CGRP较术前有增高趋势,但差异不显著(P> 0.05),而其ET 较术前低(P< 0.05);先天性心脏病患儿血浆CGRP与ET成直线负相关(r= - 0.71 P< 0.01)。结论 左向右分流型先天性心脏病患儿血浆CGRP的降低与ET的增高可能共同参与了肺动脉高压的形成。
Objective To investigate the changes of plasma calcitonin gene-related peptide (CGRP) and plasma endothelin (ET) in children with congenital heart disease (CHD) and to explore its role in the pathophysiological changes secondary to congenital heart disease and its clinical significance. Methods Twenty-nine children with left-to-right shunt with congenital heart disease were involved, of whom 11 were successfully repaired and 25 were normal controls. The levels of plasma CGRP and ET were measured by radioimmunoassay in homogeneous phase. Results The preoperative plasma CGRP in children with congenital heart disease was significantly lower than that in the control group (P <0.01), while ET was higher in the control group (P <0.01) But the difference was not significant (P> 0.05), while ET was lower than preoperative (P <0.05). Plasma CGRP was negatively correlated with ET in children with congenital heart disease (r = - 0.71 P < 0.01). Conclusion The decrease of plasma CGRP and the increase of ET in children with left-right shunt congenital heart disease may be involved in the formation of pulmonary hypertension.
