目的 :探讨骨质疏松患者股骨颈小梁微结构随骨矿含量下降所发生的变化。方法 :选择平地行走中不慎跌倒的股骨颈骨质患者 43例 ,男 2 4例 ,女 1 9例 ,受伤时间 1~ 3d。术前检查健侧股骨颈密度 ( BMD)、术中取患侧股骨颈松质骨标本在扫描电镜下观察拍片 ;按术前健侧 BMD分组、归类、排序、观察对比各 BMD组微结构变化。结果 :骨质疏松时骨小梁结构变化不仅有构成骨材料力学坚硬度的骨矿含量下降 ,而且有构成骨韧性成份的骨小梁胶原体纤维丝的破坏吸收及构成骨结构力学性能的骨小梁排列结构的变化 ,这些变化均与骨的重建偶联功能下降有关。结论 :骨的自身功能的下降是造成骨质疏松时骨的微结构变化和骨的力学强度下降的重要因素之一
Objective: To investigate the changes of femoral neck trabecular structure with the decrease of bone mineral content in patients with osteoporosis. Methods: Forty-three patients with femoral neck osteoarthritis who fell accidentally on flat ground were selected, including 24 males and 19 females, and the time of injury was 1 to 3 days. Preoperative examination of contralateral femoral neck density (BMD), intraoperative take the femoral neck cancellous bone specimens under the scanning electron microscope to observe the film; according to preoperative BMD group, classification, sorting, to observe the microstructure of each BMD group Variety. Results: The structural changes of trabecular bone during osteoporosis not only decreased the bone mineral content of the mechanical stiffness of the bone material, but also absorbed the destruction and absorption of the trabecular meshwork fibroblasts which constitute the bone toughness component and the bone which constituted the mechanical properties of the bone structure Trabecular arrangement changes, these changes are associated with the decline of bone remodeling coupling function. CONCLUSION: The decrease of the self-function of the bone is one of the important factors in the change of the micro-structure of the bone and the decrease of the mechanical strength of the bone during osteoporosis
