目的观察持续血液净化(CBP)在新生儿的应用及对脓毒症的治疗效果。方法对5例严重脓毒症的新生儿进行持续静脉血液透析滤过(CVVHDF),观察心率、血压、氧合指数的变化以及预后。结果5例新生儿均顺利置管,并进行转流,转流时间分别为1~72h,转流后心率逐渐下降,4h后恢复至正常;血管活性药物在1~3h下调,2~10h内撤除,氧合指数4h内稳定,5例中4例有效。结论持续血液净化能够在新生儿安全应用,有利于改善严重脓毒症新生儿的重要脏器功能,提高抢救成功率。
Objective To observe the application of continuous blood purification (CBP) in neonates and the therapeutic effect on sepsis. Methods Five neonates with severe sepsis were subjected to continuous venous hemodiafiltration (CVVHDF). The changes of heart rate, blood pressure, oxygenation index and prognosis were observed. Results All the 5 newborn infants were successfully placed in the tube and the time of commutation was 1 ~ 72h. The heart rate gradually decreased after the commutation, and returned to normal after 4h. The vasoactive drugs were decreased at 1 ~ 3h, within 2 ~ 10h Removal, oxygenation index within 4h stable, 4 cases in 5 cases effective. Conclusion Continuous blood purification can be safely applied in newborns, which is beneficial to the improvement of vital organs in neonates with severe sepsis and to improve the success rate of rescue.
